The mission of Community Health Center is to improve the health status of the patients and community we serve through the provision of comprehensive, high-quality, compassionate health care to all, regardless of their ability to pay.
Acerca de
Community Health Center (CHC) is a patient-centered medical home providing integrated primary care, dental and behavioral health services to the most vulnerable children, adults and families on Cape Cod.
CHC is unique in the fact that we provide care to more than 25,000 individuals within a "complete medical home" model, where multidisciplinary, integrated health services are provided under one roof to assist patients overcome transportation, affordability, financial, linguistic, cultural and other barriers to care. Patients who visit can receive primary care, behavioral health care, optometry, dental services and women’s health care, as well as specialized services and a full-service pharmacy.
