    Reports To: Director of Community Services and Director of Data Systems & Analytics

    Center for Family Life in Sunset Park (CFL) is a neighborhood-based 501(c)3 non-profit organization that aims to promote positive outcomes for children and adults through a comprehensive range of youth, family, and social services programs. Since 1978, CFL has grown with the dynamic, immigrant community of Sunset Park, offering access to resources that families need to thrive and responding to emerging needs with innovative programs including family counseling, cultural, educational, and recreational programs at neighborhood public schools, adult and youth employment programs, and an emergency storefront for food, legal and immigration services, and advocacy. In partnership with our community, we provide opportunities for personal development, enrich neighborhood residents’ quality of life, and nurture relationships that sustain families and support youth in becoming confident, capable adults.

    The Social Care Network Data Analyst will support our new Social Care Network (SCN), which aims to identify, screen, and monitor eligible Medicaid Members with Health-Related Social Needs. This position will include detailed data entry into a Medicaid-specific monitoring platform and ensuring compliance with our partner agency, Public Health Solutions, on any requests, including submission of all necessary documentation to support Medicaid billing, reimbursement, and capacity-building expenditure audits to complete all reporting requirements. Additional responsibilities include liaising with partners in our SCN, assisting in the operational set-up of our new program, and other administrative tasks related to our SCN. Detail-oriented, strong organizational skills and a focus on process improvement are key.

    I. SPECIFIC RESPONSIBILITIES include but are not limited to

    Data Analysis

    • Entry: Prepare and record outcomes for client data of Medicare-eligible members.
    • Compliance: Ensure that client data, data sharing and outcome reporting is in compliance with legal standards and into Public Health Solutions’ Unite Us Platform.
    • Monitoring: Monitor client outcomes including referral statuses and service provisions.
    • Reporting: Collaborate with our Development and Finance teams on reporting accurate outcome data on our Social Network.
    • Effectively utilize external and internal databases (e.g. ClientTrack, Client Navigator)
    • Abide by privacy laws and understand the need to maintain client confidentiality

    Social Care Network Program Onboarding

    • Assist in onboarding SCN program staff and tasks with a supervisor
    • Participate in Public Health Solutions-led training, liaison with SCN stakeholders
    • Collaborate with supervisor for timely implementation of SCN into current intake procedure

    Social Care Network Governance

    • Ensure program staff are compliant in the administration of Health-Related Social Needs screenings through full integration of the Accountable Health Communities (AHC) HRSN Screening Tool into our SCN.
    • Assist with weekly check-ins to coordinate programmatic changes or updates with SCN staff.
    • Liaison with, and if necessary, train program staff to ensure compliance with SCN Care Management.

    Medicaid Reimbursement Quality Review

    • Develop improved organization capacity for Medicaid reimbursement procedure with the Development and Finance teams.
    • Perform quality assurance review of all deliverables to ensure that only screens of eligible Medicaid Members (both FFS and Managed Care Members) would occur
    • Assist our Finance team in monitoring SCN spending as aligned with the budget
    • Recommend possible actions to improve existing processes

    Administrative Tasks

    • Respond to general tasks around SCN staff training and program implementation
    • Assist with preparing SCN-related paperwork and meetings
    • Assist with preparing contractual materials

    Outreach

    • Assist with Supervisor to devise outreach towards Medicaid-eligible clients in Sunset Park
    • Coordinate record-keeping of outreach management
    • Assist in the coordination of Health-Related public events to promote our Social Care Network

    II. QUALIFICATIONS

    • Education/Degree minimum requirement: Associate’s Degree in Computer Science preferred, or professional and community experience that may be the equivalent of or a substitute for a formal Associate's Degree
    • Bachelor's in Computer Science or relevant fields preferred
    • 2 years of Database Management and Healthcare Systems experience preferred
    • Possess an understanding or interest in Medicaid billing operations in New York State including, but not limited to, the New York Health Equity Reform (NYHER) 1115 Waiver Demonstration Amendment
    • Detail-oriented, highly organized, and able to manage time efficiently
    • Committed to maintaining and safeguarding confidential information
    • Able to work in a fast-paced environment
    • Proficient with word processing tools and spreadsheets (MS Office Word, Excel, etc.)
    • Excellent written and verbal communication skills
    • Ability to take initiative with projects and work independently
    • Bilingual in English-Spanish, English-Mandarin, English-French Creole a plus
