POSITION TYPE: Housing Retention Case Manager – Full Time Non-Exempt
DESCRIPTION: The Housing Retention Case Manager is responsible for ensuring that formerly homeless households that have been placed into permanent housing remain stably housed. The position requires regular follow up home visits, the use of Critical Time Intervention, and offers supportive services through community referrals to mental health, hospitals, and social service agencies. The goal will be to oversee highly vulnerable formerly homeless persons, ensure compliance with lease agreements, and offer additional support to ensure housing retention. The Housing Retention Case Manager helps permanently housed tenants overcome challenges related to extreme poverty, chronic homelessness, and serious behavioral and health issues. The Housing Retention Case Manager is working as part of a larger, comprehensive Case Management team within Ascencia’s Access Center.
REPORTS TO: Hospital Liaison/Wellness Case Management Supervisor
Duties and Responsibilities:
Ø Housing Retention
o Ensure smooth transition from placement to retention services;
o Implement case management service plan goals emphasizing coordinated strategies for addressing mental health/substance abuse issues, relapse prevention, medication management and any other risk factors that could impede maintaining permanent housing.
o Provide ongoing case management services that will ensure successful permanent housing, decrease social isolation and reduce relapse risks; advocacy to deter evictions; and linkage and brokerage to any needed services such as medical/dental, mental health, substance abuse, life skills training, self-help, money management, meaningful community activities, volunteer services, job placement, etc.
o Adheres to strict boundaries and professional ethics in the care of others;
o Assists clients with housing applications, completes supportive and subsidized housing paperwork, surveys rental market for affordable housing, and advocates for clients with prospective landlords.
Ø Maintain Client Records
o Maintain accurate documentation of service objectives and outcomes as well as other services in accordance with Federal, State, County and Ascencia guidelines;
o Maintain client related data systems, including case notes and complete HMIS entries;
o Prepare case management related reports including but not limited to: outcomes, successes, etc;
Ø Home Visits/Client Follow-up
o Continually monitor and evaluate each client’s progression through their housing case management plan on a monthly basis, and develop corrective action revisions to the plan as needed;
Ø Property Owner/Tenant Relations and Recruitment
o Attend program-related community, coalition and committee meetings as assigned;
o Conducts crisis intervention as needed.
Ø Case Conferencing
o Ensure effective service delivery by coordinating work with other Case Managers assigned to client.
Ø Other duties as assigned.
Qualifications and Requirements for this Position:
Four-year degree in human services or comparable combination of education/work related experience required;
Experience in social service setting with working knowledge of case management systems and planning techniques;
Knowledge of housing related resources, housing rights issues, and the Continuum of Care for homeless persons;
Experience working with homeless individuals and families a plus;
Must be proficient in the following computer applications--Microsoft Word and Excel;
Must have own transportation to conduct job related travel and must be able to drive agency vehicles.
Must meet insurance carrier’s requirements for approval to drive.
Must possess a valid driver license;
Employees must carry active and adequate auto insurance corresponding to regulatory requirements if driving any vehicle not owned or leased by Ascencia.
Physical Working Conditions and Office Setting Description
Some travel required to field locations to facilitate execution of responsibilities. Must maintain a current driver’s license and valid auto insurance with a minimum of state required levels · Requires prolonged sitting, standing, frequent bending, stooping and stretching
Some lifting of up to 25 pounds may be required
Frequent and prolonged typing, as well as operation of a computer. Use of keyboards and phones required
