    As a Supportive Housing Coordinator, you will provide direct support to CHN’s Supportive Services operations by coordinating services, education, and community referrals that empower residents and promote housing stability and recovery—serving as the primary point of contact for approximately 50 independently housed tenants and collaborating closely with Housing Facilitation, Property Management, and external service providers to address resident needs.

    What you will be doing in the role:

    • Service Coordination and Treatment Planning- Assessing tenant needs and developing individualized housing service plans; supporting housing retention through lease compliance assistance, service coordination, and regular engagement; collaborating with property management and treatment providers; and promoting economic stability by connecting residents to employment resources and benefits programs.
    • Crisis Intervention- Providing and coordinating support during resident crises using harm reduction, motivational interviewing, and crisis intervention techniques; monitoring tenant wellness through ongoing needs assessments and lethality screenings; developing safety plans as needed to reduce harm; and reviewing assessments with the Supportive Services Program Manager within 24 hours.
    • Outcome Measurement and Documentation- Providing accurate and timely documentation to ensure compliance with funder requirements, including collecting demographic and outcome data, generating service utilization and grant reports as needed, and participating in regular case reviews and monitoring to support accurate record keeping and program compliance.
    • Supportive Mediation and Advocacy- Coordinating and facilitating support team meetings for each resident; attending all scheduled partner meetings as requested; collaborating with property managers to address tenant needs while advocating for and incorporating tenant perspectives; and working with ADAMH, Housing Facilitation, and Mental Health Case Managers to identify third-party landlords and submit units to CMHA for inspection for clients using the Mainstream Voucher program.

    What you bring to the role:

    • Bachelor’s degree in social work, Psychology, Human Services, or related field
    • In lieu of bachelor’s degree, a minimum of 4 years’ experience in social services work, preferably in housing or community mental health setting.
    • 2 years’ experience in social services work, preferably in housing or community mental health.
    • LSW is preferred, not required
    • Proficient in Microsoft Office Suite Products and ability to learn and adapt to changing technology utilized in role
    • Understanding of Housing First model and Trauma-Informed Care required.
    • Proficient demonstration of Harm reduction, self-determination, and unconditional positive regard philosophies
    • Ability to communicate effectively with supervisors, tenants and CHN employees.
    • Valid Ohio Driver's license and auto insurance.
