    Job Summary: We are seeking a passionate and dedicated Part-Time Case Manager to join our team and

    support the immigrant community. This role involves providing comprehensive case management

    services to immigrants, helping them navigate various social services, advocate with stakeholders and

    work with the promoter team to share resources and coordinate care.

    Key Responsibilities:

    Client Assessment: Conduct initial assessments to understand clients’ needs, strengths, and goals.

    Resource Coordination: Connect clients with appropriate community resources, including housing,

    healthcare, education, and employment services.

    Advocacy: Advocate on behalf of clients to ensure they receive the necessary services and support.

    Cultural Orientation: Provide cultural orientation and education to help clients adapt to their new

    environment.

    Documentation: Maintain accurate and up-to-date case files, documenting all interactions and services

    provided.

    Follow-Up: Conduct regular follow-ups to monitor clients’ progress and adjust service plans as needed.

    Community Outreach: Engage with the promoter team and community organizations and service

    providers to build partnerships and enhance service delivery.
