    Description

    The mission of Circle of Friends for the Dying is to create a safe, comfortable, home-like space where individuals, with a prognosis of three months or less to live, can dwell in an atmosphere of peace and tranquility at their end of life. Cared for by trained staff and volunteers, these individuals and their loved ones can be assured round-the-clock attention to their needs.

    This is a full time position which will continue to evolve as Jim and Lisa’s Circle Home reopens its doors and becomes a fully functional Home, and operational needs become clear.

    The Home Manager will:

    • direct and manage the daily operations of Circle Home;
    • provide leadership to a collaborative and nurturing team of staff and volunteers to fulfill the mission of Circle of Friends for the Dying (CFD) and to ensure optimal care services to the Circle Home residents and their guests; and
    • orchestrate the care of residents at Circle Home in collaboration with Hudson Valley Hospice and Circle Home staff. This includes resident care and family support, staff and volunteer supervision and support, and the daily functions of the house.

    The Candidate should be knowledgeable in and understand hospice philosophy and the care culture and daily operations of a Home set forth by the Home for the Dying (AKA Comfort Care Home) care model, working with the Board, Staff and Volunteers to reach the goal of maintaining daily operations in accord with the mission of the organization. The candidate needs to appreciate flexibility and working with a small team in a residential home environment.

    Daily and General Operations:

    • Collaborate with Hudson Valley Hospice to provide care and support for residents and families, according to CFD criteria.
    • Oversee staffing and daily operations of Circle Home, which will include: employee reviews for all direct reports annually.
    • Serve as the primary liaison to the community.
    • Maintain and foster professional relationships with hospice, hospitals, medical providers, religious organizations, City of Kingston officials, and the human service community at large.
    • Review and refine programs that foster the mission of the organization in service to Circle Home residents.
    • Develop written policies and procedures for general Home operations.
    • Assist in providing direct resident care and administer medication according to the resident care plan, as needed.
    • In collaboration with the CFD Finance Committee and the Administrator, create and monitor adherence to annual operating budget.
    • Assist the Finance Committee in administering all contracts and insurance and attending to fiscal affairs which may include fundraising as needed.

    Education and Preferred Experience:

    • RN, BSN or BS in Health Care, Administration, and/or Palliative/Hospice care.
    • 3+ years experience functioning as an Administrator/Supervisor or Executive Director for a Hospice or care home facility.
    • Ability to understand financial statements and manage a budget.
