Diabetes Prevention Kidney Care

Diabetes Prevention and Kidney Care is dedicated to avert the diabetes epidemic and its cardiovascular and renal complications by providing self-management education programs directed to improve diet and exercise in the general population, and especially in minorities who are more susceptible and lack health care resources. We train Community Health Workers and patients who then follow the patients for a year.

We also provide training to primary care settings to provide self-management support to their own clinics.

We also providing nephrology consultation to individuals referred by primary care who have evidence of diabetic nephropathy and are in need of complex care.

We are in great need of fundraising expertise and membership development in order to reach effectible our target population of about 5,000 individuals at risk of diabetes and cardio-renal disease in the area.
