    Caring support for people affected by serious illness.

    VISION:A caring community making the journey easier for those affected by serious illness.

    Care Communities recruits and trains volunteers to provide emotional and/or domestic support to those affected by serious illness, including cancer and/or HIV

    Our Care Teams consist of 3-6 trained volunteers that provide companionship and share the efforts of assistance with daily living tasks (such as light housework, running errands, or going to an appointment) for a Care Partner, who is someone living with a serious illness without a strong support system. In addition, Care Partners have the opportunity to receive case management to assist with accessing the network of comprehensive health care services and other support services.
