    General Duties of F/T Case Manager Position:

    • Perform outreach at various locations throughout the state to identify and engage young adults aged 18-25 who lack or are at risk of losing permanent, stable housing or are unsheltered or unstably sheltered.
    • Provide advocacy support and refer to appropriate resources for those identified, including access to diversion funds.
    • Responsible for assigning and managing cases where YHDP funds and services are utilized. This includes but is not limited to health services, gender-affirming care, vocational and education resources, SNAP procurement, housing searches, and vetting of housing opportunities.
    • Work with peer navigators to provide education on tenants’ and workers' rights, job search support and readiness, life skills, risk reduction strategies, housing navigation, and landlord mediation.
    • Network with community groups and agencies to advance the client-identified strategies to secure housing or remain housed.
    • Provide outside referrals to agencies and programs with housing options suitable for clients in the housing referral program.
    • Some evenings and weekends will be required.
    • Staff hours will be spent at our brick-and-mortar location on Westminster Street.
    • This is not a remote or work-from-home position.

    Responsibilities:

    • Providing precise, accurate data on clients served week over week.
    • Completing intakes with youth to determine needs for housing and other services
    • Connecting individuals to the Coordinated Entry System (CES) as the main point of contact to guide youth through the process and provide advocacy as necessary.
    • Input, maintain, and monitor clients' records in the Clarity Housing Management system and YPI’s electronic health record.
    • Work with the Director of Programs and Services to link to diversion funds, mental health services, and other needed services as determined by the client’s needs.
    • Develop outreach, recruitment, and service delivery strategies with the director of youth services, the executive director, and community partners.
    • Screen, interview, and counsel young adults to determine the risk of housing insecurity
      • This includes meeting clients in the community outside of the youth center.
    • Initiate and conduct follow-up services to assure continued support, that clients' needs are met, and to determine the efficacy of those services on clients.
    • Develop, provide, and document appropriate referrals in focused areas for clients needing services.
    • Provide clients with access to transportation as needed. This may include riding with and teaching individuals to use the bus system.
    • Work with the Director of Programs and Services, case managers, and individuals served to assess the ongoing effectiveness of the Prevention, Counseling, Screening, and Referral Programs.
    • Connect to and work with other homeless diversion, support, and service programs.
    • Promote services through community outreach, street outreach, and information dissemination.
    • Attend meetings, conferences, and training to expand knowledge of low-income housing and public aid programs and be current on new developments as required by the supervisor or designee.
    • Complete all necessary paperwork and documentation in a timely and accurate fashion.
    • Assist with case management responsibilities as needed.
    • Perform duties consistent with agency/program policies and procedures.
    • Execute duties to reflect reasonable safety standards. Use universal/standard precautions and complete training as provided by the organization.
    • Collect data and complete paperwork accurately for statistical and monthly reports.
    • Perform other related work duties as needed or as required by the supervisor or designee.

    Qualifications

    • Bachelor’s degree in social work or related mental health services field, or 2+ years of experience relevant to the requirements of this position.
    • Experience working with a diverse population of youth, ages 18 to 25; LGBTQQIA2S+ youth experience preferred.
    • Lived Experience accessing public assistance (SNAP, LIHEAP, Section 8, etc), lived experience of housing instability, or people from low-income backgrounds preferred
    • Knowledge of LGBTQQIA2S+ issues, intersectionality, identity, anti-racism, community building, and other topics related to social justice.
    • Understanding of gender-and-queer-affirming care strongly preferred.
    • Strong organizational skills, including task and time management.
    • Strong facilitation and conflict resolution skills.
    • Comfortable making “cold calls” as part of resource coordination.
    • SOAR-certified or able to complete training during the first month of employment.
    • Strong interpersonal skills and the ability to work independently and in a team-oriented environment
    • Ability to adapt to a fast-changing and fast-paced environment with shifting priorities
    • A valid driver’s license is required.
    • High School Diploma, GED, or equivalent experience required
    • Bilingual in Spanish, Portuguese, Creole, or Vietnamese is strongly preferred

    Youth Pride, Inc. offers a comprehensive benefits program including three weeks of paid vacation time, 40 hours of sick time, 15 paid federal holidays, and two paid weeks of center closures - the week of the 4th of July and the week between Christmas and New Year's Day. You can choose between Blue Cross Blue Shield or Neighborhood Health Plan for health and dental (bronze and silver tiers available), or a yearly reimbursement of $2,399.80 ($46.15 per weekly pay period) if you do not utilize our insurance. We offer vision insurance through SunLife for an additional $8-$15 per pay period. We reimburse travel at the current federal mileage reimbursement rate of $0.67 per mile.
