    CASE MANAGER - RESOURCE COACH

    Background:

    Food on Foot is a nonprofit dedicated to assisting our unhoused and low-income neighbors in Los Angeles with nutritious meals, clothing, and a fresh start through life-skills training, full-time employment, and permanent housing.

    Definition:

    The Resource Coach Team Leader is a case management specialist, responsible for providing case management and employment services to Program Members who are experiencing homelessness as well as providing thorough evaluations on program success. While this work is often accomplished by the Resource Coach Team Leader, the role also includes developing and training community volunteers who can work together to provide the bulk of direct member services. The Resource Coach advocates for meaningful rehabilitation, employment, and stabilization of Program Members through assessments, evaluations, supervision and overall support, and leads a team of volunteers in direct service. This is a dynamic service role including many offsite and outdoor interactions with Program Members experiencing homelessness.

    The Resource Coach reports to the Director of Programs.

    Duties:

    • Conducts, intakes, and screens current Program Members and Program applicants. Tasks include: Surveying current living situations, mental stability, personal background and family data, along with drug screening.
    • Compiles, analyzes, evaluates, and reports to the Director of Programs on all information obtained during weekly and daily meetings with Program Members.
    • Ensures the development of appropriate goals and benchmarks, as well as formulating specific recommendations using comprehensive knowledge of all relevant laws and local resources that assist Program Members in moving step-by-step from homelessness to employment and permanent housing.
    • Ensures that all Program Member case notes are kept and appropriately logged in the database.
    • Ensures Program Member commitment and follow-through by developing supervision and treatment plans that include identification of individual needs, establishment of specific objectives, and plans to meet those problems and objectives.
    • Supervises Program Members through direct meetings and phone communication while implementing supervision plans, providing counseling, and making referrals to other social agencies.
    • Maintains regular contact with all current Program Members and ensures that care plans are being achieved including job search and job performance, financial savings, housing search and housing retention, and more.
    • Provides outreach to individuals experiencing homelessness as well as key social services organization, encouraging individuals to enroll in Food on Foot programs and other services for housing, victim support, healthcare, and other identified needs, and documenting all outreach.
    • Provides employment education services to formerly unhoused individuals.
    • Secures ongoing relationships with community service providers.

    Qualifications:

    • Bachelor's degree from an accredited college or university required. (May substitute the bachelor’s degree with four-years' experience in a case management role).
    • Minimum three-years' of specialized experience working directly with unhoused or at-risk adult populations.
    • Demonstrates expertise in Microsoft Office Suite and an ability to quickly learn and implement additional client-related software and reporting packages.
    • Outstanding written and verbal communication skills.
    • Ability to manage, support and retain community volunteers.
    • Reliable transportation and clean driving record

    • Position consists of a 40-hour work week. Must work on Sundays. Exact hours to be
    • determined. Full-Time.

    Salary: $52,000 - $60,000 per year
