    At Values Into Action, we partner with adults with intellectual and developmental disabilities to live full, meaningful lives in their own homes and neighborhoods. We provide this support on a one-to-one basis with people supported 24/7. We’re looking for a Community Support Facilitator who is ready to coach, collaborate, and champion the voices of the people we support and the teams that walk alongside them.

    About the Role:

    As a Community Support Facilitator, you’ll provide leadership and mentorship to a team of Support Partners, ensuring that the people we support are at the center of every decision and action. You’ll guide your team in delivering high-quality, respectful, and empowering support—always aligned with the individual’s goals, preferences, and rights.

    Key Responsibilities:

    • Mentor and supervise Support Partners, fostering a team culture of accountability, respect, and continuous learning.
    • Support individuals to lead their own lives and direct their own teams, including participation in planning and decision-making.
    • Ensure health, safety, and rights are upheld in all aspects of support.
    • Promote community inclusion and natural support networks, helping individuals build meaningful relationships.
    • Facilitate team meetings and planning processes, ensuring the person supported is actively involved.
    • Provide direct support when needed, modeling best practices.
    • Participate in the on-call rotation in support of 24/7 teams
    • Assist with staffing, scheduling, and recruitment, in collaboration with the person and their team.
    • Encourage the use of assistive technology and communication supports to enhance independence and self-expression.
    • Maintain compliance with service standards and documentation requirements.
    • Celebrate successes and support growth, both for the people supported and your team.

    What You Bring:

    • A strong belief in person-directed support and inclusive communities.
    • Experience in disability services, social work, or a related field.
    • Excellent communication, coaching, and problem-solving skills.
    • A collaborative mindset and the ability to build trusting relationships.
    • Familiarity with regulatory standards and commitment to ethical practice.
    • Flexibility, reliability, and a sense of purpose.

    Requirements

    One of the following:

    • A master’s degree or above from an accredited college or university and 1 year’s work experience working directly with persons with an intellectual disability and/or autism.
    • A bachelor’s degree from an accredited college or university and 2 years' work experience working directly with people with an intellectual/developmental disability and/or autism.
    • An associate’s degree or 60 credit hours from an accredited college or university and 4 years' work experience working directly with persons with an intellectual disability and/or autism.
    • A high school diploma or general education development certificate and 6 years' work experience working directly with persons with an intellectual disability and/or autism.

    Candidates with NADSP credentialing are encouraged to apply! We offer a bonus to candidates who have current NADSP (DSP or FLS) credentialing and complete their first six months with Values Into Action.
