POSITION SUMMARY:
Identity, Inc. is seeking a dynamic, responsible, detail-oriented, and flexible individual who enjoys working with youth ages 11-14 to be a therapeutic mentor. We serve youth and their families living in Clarksburg, Damascus, Germantown, and Poolesville. The therapeutic mentor will work closely with the Clinical Supervisor and the Program Coordinator to assess and develop individualized service plans to address behavioral or social skill building in youth. This program pairs youth with one therapeutic mentor for one-on-one skill building, coaching, and practice to address behavioral concerns.
PRIMARY RESPONSIBILITIES:
- Works closely with the Clinical Supervisor and Coordinator to assess and develop individualized service plans for each client. Service plans will contain specific, measurable, achievable, relevant, and time-bound (SMART) goals related to the areas of language/communication, attention and working memory, emotion and self-regulation, cognitive flexibility, and social thinking.
- Establishes a positive and trusting relationship with the youth, families, service providers and school staff to coordinate seamless care for the client.
- Must respond to clients, providers, families, and other staff within 48 business hours.
- The therapeutic mentor meets weekly with clients individually or in small groups to work on their goals, mentor, coach, and practice skills so youth can find success.
- Provides real-life opportunities for youth to practice their new skills with peers, school staff, and family members.
- The therapeutic mentor is responsible for engaging with youth and making referrals to positive youth development programs, or other services they may need. They follow up with providers to ensure that clients receive services and keep meticulous documentation of treatment plans, interventions, and external referrals provided to each client.
- Facilitates referrals to Identity’s Substance Use Prevention/Intervention Program, middle school programs, family strengthening/reunification, and case management programs to ensure a warm hand-off and make sure that youth have received services.
- Troubleshoots in times of crisis, assists when therapeutic mentors need assistance, and helps to connect mentors to services and programs that are needed by their clients.
- Participates in clinical supervision in person or online to discuss progress on cases and to review cases at three and six months to determine if client is ready for discharge.
- Ensures that enrollment, consent, and other treatment forms are completed within 10 days of referral. All efforts with youth and families must be documented through the Montgomery County DHS electronic records systems and enter the data within two workdays of the date of service was delivered.
- Ensures that youth and parents receive a customer satisfaction survey upon discharge from services and that those surveys are received by the Evaluation & Quality Assurance Team.
- Attends County, school, and partner meetings with coordinator or director to provide information, updates, and outreach to various stakeholders in a professional, positive, and respectful manner as a representative of the program.

EDUCATION AND EXPERIENCE:
- Must possess a bachelor’s degree in human services or a minimum of three of years experience doing similar work with youth.
- Training or knowledge in at least two of the following: trauma informed care, stages of change, motivational interviewing, substance use disorder, opioid use disorder, harm reduction strategies, dialectical behavioral training, or functional behavioral plans are desired.
SKILLS AND ABILITIES:
- Fluency in oral, written, and presentation skills in English is required. Fluency in another language is a plus.
- Must be able to pass a federal fingerprinting and background check.
- Position requires travel between sites and access to a car, valid driver’s license and a clean driving record. Mileage reimbursement is available in this position.
- Must complete the following training upon hire: Advancing Youth Development, SBRIT training, Motivational Interviewing Training, Sexual Harassment Prevention Training, Child Protective Services, HIPAA, CPR/First Aid, Collaborative Problem Solving, etc.
- Must be proficient in MS Office Suite (word, PowerPoint, excel, forms, Teams, etc.), Adobe, Canva, and Zoom.
- Identifies each youths’ strengths and works from an asset-based approach to help build rapport and trust with young people.
- Demonstrates ability to be a positive, open-minded, and collaborative team member who enjoys working towards one goal and enjoys learning from others.
- Demonstrates sound judgment and decision making when working with colleagues, youth, families, and partners. Shows flexibility, initiative, and resourcefulness in all actions and conversations.
- Possesses the technical knowledge and abilities to perform job responsibilities while demonstrating a high skill level as well as reliability.
WORK ENVIRONMENT:
- This is a full-time, 40-hr a week work schedule, 60% (24 hrs) of the work week must be engaging with youth face to face at times that they are available including afternoons, evenings, or weekends.
- This position is eligible for remote work one day a week, after 90-day probationary period, if performance meets expectations.
- When working remotely staff are required to have consistent, full-time access to Wi-Fi/Internet and a quiet working environment. Must be available to answer emails, phone calls, and Teams chats during scheduled working hours and come to the office if needed.
- Must be flexible during the workday to move from remote location to on-site/schools/office as needed to serve the needs of staff and clients.
- The ability to lift and move up to 10 lbs.
