    Description

    Position Summary: The Family Coach is responsible for working with families during their stay in CCA’s shelter and for up to 12 months after exit to promote their self-sufficiency, using a person-centered approach that is designed to meet the comprehensive needs of families that are homeless.

    MAJOR DUTIES AND RESPONSIBILITIES

    • Coordinate Service Delivery Improvement Initiative (SDII) resident-driven goal planning meetings
    • In partnership with the Head(s) of Households (families) and for coordination of services, become involved in their own planning and goal setting and development of a family-driven service plan
    • Complete the initial intake and act upon critical needs, appropriately and immediately, for CCA’s Hillside Family Shelter (HFS)
    • Coordinate SDII resident-driven goal planning meetings
    • Orient Head(s) of Household to the HFS shelter requirements
    • Work in partnership with the Head(s) of Household to plan and set goal setting and develop a family-driven service plan
    • Meet regularly with Head(s) of Households to discuss goals and objectives, service plan progress, and challenges and barriers
    • In partnership with the Head(s) of Households and ARISE staff, develop the Family Service Plan and update the service plan every 3, 6 and 9 and 12 months or as necessary
    • Provide follow up to families up to a year after they move into permanent housing
    • Refer Heads of Household to appropriate resources to meet their stated goals
    • Maintain files in HMIS and hard copy case file including conversations and progress notes towards achievement of goals
    • Collect data and statistical reports that are necessary to meet funding requirements
    • Enter all data and upload required documents into HMIS
    • Maintain shelter bed nights in HMIS as needed.
    • Collect proof of income and document
    • Work with Director of Housing and Resident Services to assure that specific funder program goals are consistently being met

    OTHER DUTIES

    • Attend monthly Housing Team and agency wide staff meetings
    • Attend monthly CCA Case Consultation meetings.
    • Attend Coordinated Access Network (CAN) Family Housing Case Consultation meetings as required.
    • Attend CAN daily morning call
    • Attend bi-weekly HFS Tenant meetings
    • Represent CCA in the community (as requested)
    • Maintain positive working relationships with agency staff and community partners
    • Respond to informational inquiries and collaboration invitations

    KEY QUALIFICATIONS AND CAPABILITIES

    • Bachelor’s degree in social work, human services, or related field
    • Experience working with particular target populations, including persons experiencing homelessness
    • Demonstrated success working with diverse populations - especially in low-income neighborhoods
    • Ability to communicate in Spanish
    • Ability to resolve conflicts effectively and solve problems in situations of immediate crisis or conflicts
    • Ability to coach individuals to develop goals
    • Demonstrated success with developing and managing strategic partnerships
    • Ability to build rapport while maintaining professional boundaries
    • Experience with facilitating groups
    • Ability to work cooperatively within a team
    • Strong interpersonal and time management skills
    • Demonstrated written and verbal communication skills
    • Proficiency with Microsoft Office (e.g., Office, Excel, PowerPoint, and Publisher)
    • Willingness to comply with all CCA policies and procedures
    • Ability to work non-traditional hours, including evenings and some weekends
    • Reliable transportation for meeting with Heads of Household
    • Experience with public speaking
    • Training in Trauma Informed Care, Motivational Interviewing and Person-Centered Practice.

    STAFF COLLABORATION AND INTERDEPENDENCE

    • Work with ARISE staff to identify topics for year-long training schedule
    • Participate in case consultation team meetings and adheres to group decisions
    • Demonstrate effective communication skills in building relationships with all employees and clients
    • Create and maintain good working relationships with other area service providers and support groups to facilitate access to area resources for the clients
    • Substitute for other staff when the need arises
    • Work in partnership with the ARISE Center, building superintendents, and report directly to the Director of Housing and Resident Services. Each Family Coach will assess for needs and screen the people who are served by CCA to determine any other referral services that may be needed.

    REPORTING REQUIREMENTS

    • Meet with CCA’s Director of Housing Services on a monthly basis for supervision or as needed
    • Submit timesheets in a timely manner
    • Submit monthly family updates to the Director of Housing and Resident Services
