Social Worker

Salary Range:$60,000.00 To $70,000.00 Annually

The Social Worker – Medical Respite Program plays a vital role in providing trauma-informed, person-centered support to young adults experiencing housing insecurity alongside medical challenges particularly those facing maternal health complications or undergoing cancer treatment. This position is part of a new initiative launched in partnership with New York State under the Medicaid waiver, offering medical respite housing and supportive services to up to 10 patients and their caregivers or family members at a time.

The Social Worker – Medical Respite Program is responsible for conducting intake assessments, developing individualized care plans, facilitating service navigation, and monitoring participant progress. This role will report to the Medical Respite Program Manager and involves close collaboration with internal team members and external partners to ensure comprehensive, coordinated care. At times, the Social Worker may also support families staying in the organization’s core housing programs and is expected to actively contribute to a culture of teamwork, empathy, and high-quality care across the Family Support team.

The salary range for this role will be 60K-70K. The schedule is Monday through Friday, 9 AM – 5 PM with some flexibility and weekends as needed.

Role/Responsibilities

Case Management

• Conduct SDOH assessments upon intake and develop individualized service and support plans.
• Provide trauma-informed, strengths-based support and care coordination.
• Facilitate service navigation across medical, housing, mental health, benefits, and community support systems.
• Monitor progress, adjust care plans as needed, and maintain regular contact with participants throughout their stay.
• Advocate for participants’ needs within health, housing, and social service systems, helping to reduce barriers to access.
• Identify and address systemic gaps or inequities impacting participant well-being, escalating concerns when appropriate.

Collaboration & Team-Based Care

• Collaborate with internal staff and external partners to ensure coordinated, holistic care across disciplines.
• Contribute to team case reviews, multidisciplinary planning, and continuous quality improvement efforts.
• Partner with on-site and off-site social workers, medical professionals, housing navigators, and support staff to ensure coordinated, wraparound support.

Documentation & Compliance

• Maintain accurate and timely case notes, assessments, and reporting in compliance with Medicaid and organizational standards.
• Support data collection and reporting efforts for funders, audits, and program evaluation.
• Uphold HIPAA and all relevant confidentiality and regulatory standards.

Program Support & Coverage

• Participate in 24/7 staffing coverage model, which may include occasional evening or weekend on-site support.
• Serve as backup support to the broader Family Support team as needed.
• Assist with participant transitions, crisis response, and discharge planning.

Qualifications & Competencies

• MSW (Master of Social Work) from an accredited institution.
• LMSW required; LCSW preferred.
• Minimum 2 years of relevant experience working with individuals facing housing insecurity, maternal health challenges, or chronic illness.
• Demonstrated understanding of trauma-informed care, housing navigation, and Medicaid programs.
• Strong interpersonal, communication, and organizational skills.
• Commitment to equity, dignity, and culturally responsive care.

Preferred:

• Experience working in a residential or respite care setting.
• Familiarity with NYC social service and housing systems.
• Bilingual (English/Spanish or other relevant languages) a plus.

• This job is located in the same facility where RMH-NY provides temporary accommodations to pediatric patients and their families while the patients are undergoing active medical treatment at area hospitals. For some RMH-NY guests, medical treatment may include radiation therapy. While radiation therapy is never administered at RMH-NY, there is the potential for some radioactive material to be present in guests who have undergone radiation therapy. Based on radiation measurements performed throughout RMH-NY, it is not expected that individuals working at RMH will receive radiation doses that would exceed the general public limit (100 millirem per year).
• The noise level in the work environment is moderate to loud. Noise levels often increase with families and children who have temporary residence in the building as well as guests and musical performers for special events as well as mechanical equipment
