Under the supervision of the Director of the Memory Center, the Social Worker will assist older adults with cognitive decline, and their caregivers, access resources, provide referrals and benefits/entitlements on a range of supportive services; in partnership with community stakeholders, advocate and intervene on behalf of our constituency; compile data and maintain accurate case records to remain compliant with funder mandates; in addition, with assistance from key staff members, assist with recruitment and community outreach efforts.
Responsibilities:
- Conduct comprehensive assessment with older adults and caregivers to assess psychological needs, as well as case assistance for eligibility on benefits and entitlements (Medicaid, Managed Long Term, Home Care, SSI, SCRIE, Food Stamps, Section 8) and determine the nature and degree of need; conduct home visits as needed;
- Complete and maintain up to date written case notes including admission, suspension, care and discharge planning;
- In conjunction with other staff members work on special projects such as surveys, research projects, and caregiver events;
- Conduct Care Giver Support groups in Spanish;
- Keep accurate agency and funding source(s) required documentation(s)
- Attend trainings, workshops and case conferences as required to maintain appropriate knowledge of industry trends and policy changes;
- Conduct activity groups for program participants when needed;
- Assist with arranging and monitoring transportation on behalf of program participants, and perform miscellaneous job-related duties as assigned by the Director;
Required Qualifications:
- BA in the field of Social Work or human services; and at least 1 year of work experience preferably in the field of aging; Associate Degree with measurable experience in the field of aging will be considered.
- Bilingual in Spanish is required;
- Experience with or willing to learn advocacy skills; benefits & entitlements and referral systems, and outreach/recruitment;
- Ability to communicate with empathy and sensitivity with program participants, caregivers, and related staff;
- Demonstrate excellent organization and communication skills;
- Good record-keeping abilities and proficiency in computer/web-based programs;
