    Description

    Job Definition: This role is the principal contact at the local school site and will coordinate programs in tandem with the school’s principal, ensuring that program performance measures are implemented at the site level. The individual has a solid understanding of community resources, can function effectively in a demanding work environment, and can navigate multiple systems (non-profit sector, education, legal, and child welfare). Furthermore, the individual is committed to working with urban youth and families identified as high-risk and can demonstrate a sufficient level of trauma-informed care and cultural competence.

    Essential Functions/Responsibilities:

    • Strong organizational, communication, and written skills.
    • To serve as the site-based team leader
    • Serve on School Based Leadership Team(s) representing the Community Partner.
    • Organize and oversee a community resource and needs assessment process.
    • Provide timely feedback to referral sources
    • Supervise a diverse staff of multi-industry backgrounds, ensuring their adherence to agency processes and protocols.
    • Facilitate cross-referral of students and families between service providers
    • Facilitate ongoing communication between service providers, teachers, parents, and students
    • Recruit and coordinate the activities of individuals and organizations willing to offer programs and services at the school to meet the need of student, families, and community members
    • Participate in the development, implementation, and oversight of Family events, designed to advance the mission of the school and agency.
    • Coordinate and provide coverage and on-site support for community partners.
    • Provide individual, group, and/or family support to students and family members
    • Maintain a client caseload that meets the agency’s level of service.
    • Oversee the development of Case Management, ensuring, family/student support plans, informal assessments, progress/group notes, and other activities are followed to ensure proper documentation for each client.
    • Maintain confidentiality of records related to clients’ case management
    • As appropriate and permitted, communicate with families, parole officers, police, and other interested parties to exchange necessary information during the case management process and make referrals for other needed services.
    • Complete and maintain monthly reports, data bases and other paperwork as required by agency.
    • Attend necessary onsite and external meetings, as needed.
    • Respond to emergent situations and interpersonal conflict in a calm, professional manner.
    • Adhere to highest standards of professional conduct.
