    The Emma L. Bowen Community Service Center is seeking an Assistant Director -Adult & Children's CCMP and HHC Health Homes to join our team. This position is a terrific opportunity for someone to gain additional experience and make a difference in the lives of our clients.

    For over 45 forty-five years, our organization has provided mental health and holistic services in northern Manhattan for families, children, adolescents, and other adults including the elderly in a supportive and caring environment. We also have an out-patient and residential substance abuse program as well as a Therapeutic Preschool for 3-5 year olds.

    Upper Manhattan Mental Health Center is a Certified National Health Service Corps (NHSC) site.

    JOB DESCRIPTION

    Position Summary

    The Assistant Director for Adult and Children’s CCMP and HHC Health Home Care Coordination Management Program provides leadership, oversight, and operational support to ensure high-quality, person-centered care coordination services across the lifespan. This role is responsible for supervising care management teams serving both adult and children's populations, ensuring compliance with state, federal, and contractual requirements, and collaborating with healthcare, behavioral health, child welfare, and community-based partners to improve health outcomes. The Assistant Director will also assist in program development, staff training, performance monitoring, and strategic initiatives that align with organizational goals.

    Key Responsibilities:

    _ Program Oversight & Leadership_

    • Provide day-to-day management and leadership for Adult and Children’s CCMP and HHC Health Homes programs.
    • Support the Director in implementing policies, procedures, and workflows to ensure compliance with DOH, Medicaid, and managed care requirements for both adults and children.
    • Monitor service delivery to ensure care management teams are meeting performance benchmarks, quality indicators, and engagement goals across age groups.
    • Analyze program data and prepare reports to support decision-making and quality improvement initiatives.

    _ Staff Supervision & Development_

    • Directly supervise Program Supervisors, Team Leads, and Care Managers working in both Adult and Children’s programs.
    • Conduct regular team meetings, case conferences, and performance reviews.
    • Identify training needs and provide coaching, mentoring, and professional development opportunities.
    • Support staff with problem-solving complex adult and pediatric cases, including coordination with families, schools, child-serving systems, and healthcare providers.

    _ Quality Assurance & Compliance_

    • Ensure program adherence to all regulatory, contractual, and organizational standards for both adult and children’s services.
    • Implement quality improvement initiatives to enhance service delivery and client outcomes.
    • Monitor documentation, billing practices, and data entry to ensure accuracy and timeliness.
    • Support audits, site visits, and reporting requirements from funders and regulatory agencies.

    _ Collaboration & Community Partnerships_

    • Foster strong relationships with healthcare providers, pediatric specialists, schools, child welfare agencies, behavioral health providers, and community-based organizations.
    • Represent the agency at external meetings, workgroups, and community forums as assigned.
    • Collaborate with internal departments to ensure seamless service delivery and resource coordination across programs.

    _ Administrative & Strategic Support_

    • Assist the Director in developing program budgets, staffing models, and expansion strategies for both Adult and Children’s CCMP and HHC Health Homes.
    • Track and evaluate program performance, identify gaps, and recommend enhancements.
    • Participate in grant writing, proposals, and contract renewals as needed.
    • Support the integration of evidence-based practices and innovative care coordination models.

    Qualifications:

    _ Education & Experience_

    • Master’s degree in Social Work, Public Health, Nursing, Psychology, Human Services, or a related field required.
    • Minimum 5 years of experience in care management, case management, or health/behavioral health services with both adult and/or children's populations.
    • At least 2–3 years of supervisory or management experience in healthcare, child welfare, community health, or human services.
    • Knowledge of Health Homes, Medicaid, managed care, and care coordination models for both adults and children strongly preferred.

    Skills & Competencies

    • Strong leadership and team management skills with the ability to motivate and develop staff.
    • Excellent organizational, analytical, and problem-solving abilities.
    • Proficiency in program monitoring, reporting, and quality improvement practices.
    • Strong interpersonal and communication skills with the ability to engage diverse populations, including families and children.
    • Computer literacy, including experience with EHRs (Foothold/EPIC Carelink), care management systems, and Microsoft Office Suite.

    _ Working Conditions_

    • Hybrid of office, remote, and field-based work as required.
    • Occasional evenings or weekends may be required based on program needs.
    • Travel to sites, community partners, and meetings within the region.

    Salary & Benefits

    • Salary Range $80,000 - $90,000 per year
