The Position
The Senior Program Manager at the Learning Center will take the lead in developing and implementing new programs, focusing on key areas of Population Health Management (PHM). This includes utilizing data and technology to improve patient outcomes and reduce disparities, collaborating with managed care plans to implement their PHM strategies under CalAIM, and optimizing our learning management platform to scale our curriculum across various programs and primary care practices. Reporting to the Director of Programs, the Senior Program Manager will play a key role in shaping, executing, and refining strategies that support California's safety net providers in advancing equitable health outcomes.
Candidates must have demonstrated experience navigating the California healthcare landscape, including familiarity with Medi-Cal, Medicaid managed care plans, DHCS programs, and/or CalAIM initiatives, to effectively collaborate with safety net providers and managed care partners.
The ideal candidate will have experience in designing and implementing large-scale primary care transformation initiatives, leading multi-stakeholder efforts, and leveraging technology such as EHRs and PHM systems to enhance population care. Additionally, the candidate should have expertise in utilizing Quality Improvement (QI) methods to inform program design, execution, and continuous improvement.

Compensation & Benefits
The salary range for this position is $140,000 - $180,000. PHLC offers a generous benefits package that reflects our commitment to team member health and wellness.
