Job Summary:
La Raza Community Resource Center (La Raza CRC) is a bilingual, multi-service non-profit organization dedicated to meeting the social services, immigration, educational, and leadership development needs of low-income families and individuals. La Raza Community Resource Center is located in the heart of the historic San Francisco Mission District, the vibrant cornerstone of San Francisco's Latino community. We have been serving the community for over 50 years. Our Social Services Program services include a food pantry, family counseling, educational workshops, support groups, rental assistance, and many other services. Our Immigration Program attorneys provide both affirmative family-based immigration services and removal defense. All of our legal and social services are free (subsidized).
The Family Resource Services Case Manager is passionate about working with immigrant families and adopts a strengths-based, non-judgmental approach. This job requires the ability to build meaningful relationships with a diverse range of individuals and agencies, with emphasis on identifying basic needs support such as food security, housing support and family resource services. It also requires a deep understanding of the unique challenges faced by these families, the ability to offer trauma-informed case management, and the experience to provide a range of community and family support services. The Case Manager is responsible for ensuring that the standards of quality for family strengthening and support framework are implemented. This includes conducting a comprehensive needs/strengths assessment and service plan, monitoring the progression of the service plan, and assessing the quality of the social services provided to families.
Responsibilities:
Case management and General Duties
- Provide trauma-informed and person-centered case management support that includes consistent engagement to families, at least two contacts a month but preferably weekly, assisting families in developing the skills and knowledge to eventually access resources independently.
- Completes formal intake, and needs assessment, and facilitates planning process to assist families in developing individualized service plans (ISPs) that address their specific needs and goals and concerns impacting child development and family functioning.
- Link Families to essential services, such as social and health services, housing assistance, food security programs, job training, and financial assistance.
- Advocates on behalf of families to help them navigate systems for education, health, employment and social services.
- Maintains accurate and up-to-date client files, service plan, progress notes, referral log, sign-in sheets, evaluations, enrollment forms, client satisfaction surveys, assessments, incident reports, and data in computerized case management systems, ensuring compliance with program requirements, and ensuring confidentiality of all family records.
- Conduct consistent monitoring to review progress towards identified goals. Services can be short or long-term, the last being at least 6 months or longer.
- Attends and participates in all scheduled staff meetings and training, collaborates with community partners and other stakeholders, and conducts community outreach to promote program services.
- Collaborate with other agencies, advocating on behalf of families to coordinate care and follow up to ensure the families accessed the services or located a different option.
- Completes all required reports, forms, and documentation for family and community partnerships timely and consistent.
- Works in tandem with the parents, staff, and community representatives to coordinate family services and meet all Standards of Quality for Family Strengthening and Support Framework.
- Complete monthly data entry and monthly summary into the city electronic management system.
- Provide referrals to other city agency or nonprofit resources, and interdepartmental programs to meet the needs of clients.
- Participate in informational community meetings, workshops and training with community partners and stakeholders related to community services either in person or remotely.
- Assist with the daily operations of the food market, which may include supervising food distribution, managing inventory, and ensuring a positive client experience.
- Answer questions and provide assistance to clients related to food market issues and eligibility.
- Supervise a team of 10-15 volunteers on a weekly basis. Help coordinate and recruit volunteers.
- Identify local and state funding sources to enhance food recovery and distribution.
- Meet with the role’s supervisor on a weekly basis to report, problem-solve, and plan regarding position responsibilities.
- Ability to interact positively with team members, and facilitate positive change.
- Partner with the supervisor when necessary to address department issues or concerns.
- Participate in community advocacy on behalf of the agency (i.e. public comment, budget advocacy, and other related tasks).
- Other tasks as assigned.
Qualifications:
- Experience supporting families overcoming barriers, such as housing, immigration, child safety, family functioning/relationships, parenting, health, mental health, substance abuse, socio-economics, child school success, and/or well-being.
- Strong communication and interpersonal skills to build trust with families and effectively collaborate with various programs and agencies.
- Cultural humility and a strength-based approach to client-centered service.
- Organizational skills for managing a caseload and maintaining detailed documentation.
- Advocacy skills to represent families’ interests and ensure they access appropriate resources.
- Patience, compassion and empathy when working with families experiencing various challenges.
- Stress Management - Ability to practice self-care habits and emotional regulation to notice earlier signs of burnout and communicate with supervisor.
- AS Degree with an emphasis in Social Work, Counseling, Psychology, Human Services, Family Development, Family Studies, or 3 years of relevant experience
- Bilingual English/Spanish fluency, both oral and written.
- 3+ years’ of community-based direct service experience 1+ years of Case Management Experience
- Proficiency in Microsoft Office applications: Outlook, Word, Excel, and PowerPoint.
