    Organizational Overview:

    DelgraciaCorp is a 501(c)(3) nonprofit organization founded in 2017 to support young parents and their children aging out of foster care or at risk of child welfare involvement. Our mission is to preserve young families using socioeconomic opportunities that empower and safeguard individuals and families’ social-emotional needs. Our Family-Based Care Management (FBCM) Program offers six months of wraparound services, including:

    • Preventive Case Management
    • Brief Mental and Behavioral Health Counseling
    • Peer-to-Peer Counseling
    • Family Therapeutic Recreational Activities

    Position Summary:

    The Master Social Worker (MSW) will play a vital clinical and facilitative role in delivering trauma-informed care to young adults with children in transition.

    This position combines individual short-term therapy, group facilitation, and curriculum development for clients impacted by the child welfare and justice systems.

    Key Responsibilities:

    Clinical Services:

    • Maintain a therapeutic caseload of 5–10 families.
    • Conduct intakes, mental health assessments, and develop treatment and discharge plans.
    • Provide short-term psychotherapy using an eclectic, evidence-based approach (trauma-informed practice, solution-focused brief therapy).
    • Document all services (progress notes, assessments, group attendance) in the EMR within 24–48 hours in compliance with state, federal, and agency guidelines.

    Group Facilitation:

    • Co-facilitate a bi-weekly, open-group therapy series (clients can join at any point in the cycle).
    • Establish group norms and expectations in collaboration with participants to create a safe, inclusive space.
    • Provide crisis intervention and management as needed during sessions.

    Leadership & Team Collaboration:

    • Participate in weekly individual clinical supervision including recommended trainings.
    • Attend and contribute to interdisciplinary team meetings and case conferences.
    • Serve as a community liaison and advocate for families.
    • Communicate ongoing concerns or clinical observations to the Care Manager and Executive Director.

    Administrative and Quality Assurance:

    • Support the development and updating of mental health policies and procedures.
    • Participate in quality improvement initiatives and internal audits.
    • Submit bi-weekly service reports and client progress summaries.
