    Description

    Bereavement Support Volunteer

    The Bereavement Support Volunteer serves as an additional support to the bereavement program, which provides services to grieving families and caregivers during the 13-month period following the death of the hospice patient. Support by the volunteer may be provided in a variety of ways, including assisting with administrative tasks (such as standard mailings) or facilitating/assisting with a Bereavement Support Group and/or assisting with the Annual Memorial Service. If used in a “counselor” role, the volunteer must have documentation of qualifications/education/experience to provide bereavement support/counseling (for example, counseling provided to the bereaved during support groups or memorial services). All support provided by the Bereavement Volunteer is under the direct supervision of the Bereavement Coordinator (or designee). All assignments provided to the Bereavement Support Volunteer must be pre-approved and assigned by the Bereavement Coordinator (or designee).

    Volunteer Prerequisites

    1. Be receptive to all aspects of the hospice concept.
    2. Complete a Volunteer Application, to include two personal or professional references.
    3. Complete a Certiphi Application authorizing the hospice to request a background check.
    4. Urine drug screen required.
    5. Recognize the necessity of commitmnet and confidentiality and sign a statement verifying agreement with this policy.
    6. Two-step Tuberculosis skin test (PPD) at no cost to the volunteer. Proof of PPD results with the last 12 months may be accepted for one of the required tests.
    7. Sign a consent/declination for the Hepatitis B vaccination. This vaccination (consisting of 3 shots) is available at no cost to the volunteer if he/she chooses to recieve it.
    8. Be of good physical and emotional health.
    9. Have reliable transportation.
    10. Complete Hospice Volunteer training and Bereavement Volunteer Training.

    These prerequisites are for the safety of our patients and volunteers.

    Instructions:

    Once contact information is received, a Gentiva Volunteer Coordinator will contact applicant by phone. A link will be sent to the provided email to initiate the background check and drug screening process. Once this process is started, a meeting will be scheduled to sign required documents. Training will also start at this time as well, and may take 2-3 meetings to complete, depending on the volunteer's schedule. Once training and screening is completed, the volunteer will be able to begin assisting with our Bereavement Program.
