    Care Management Support Volunteer

    Support the Team Connecting Patients with Post-Discharge Services

    Our care management team connects our patients with their services they need after their discharge. By joining this team, you be a vital link between care management, providers, and patients. You will answer the phone, call patients with follow-up information, and other duties to keep the office running smoothly.

    This role is for you if you:

    • Enjoy working behind the scenes to support patients
    • Comfortable making and receiving phone calls
    • Relish in keeping an office organized
    • Available once a week from 9:30 am - 1:30 pm

    No experience necessary! We will train you on everything you need to know to be successful.

    1 shift available:

    • 09:30 to 13:30 CST
