    This job description is a broad outline of the responsibilities and duties of the Housing Support Specialist and will be amended, as needed, to meet the program priorities of Women Aware, Inc. (the “Agency”) and its ensuing work requirements. It should not be construed as a contract and does not create a contract of employment. Employment with the Agency is at will.

    Reports to: Director of Residential Services

    Status: Non-Exempt

    Qualifications:

    • Bachelor’s degree in social work, Human Services, Public Health, or related field preferred
    • 1–3 years of case management experience
    • Demonstrated work with individuals experiencing homelessness or housing instability
    • Knowledge of Medicaid programs, benefits, and care coordination requirements
    • Experience with Medicaid billing and service documentation
    • DV experience preferred
    • Strong crisis intervention skills
    • Bilingual Spanish a plus
    • Full-time position with some evening availability required
    • Driver’s license: clean and valid
    • Lifting up to 25lbs

    General Responsibilities:

    In collaboration with the Director of Residential Services, the Housing Supports Specialist is responsible for providing intensive case management services to individuals and families receiving or eligible for Medicaid, who are experiencing homelessness or housing instability. This role focuses on helping clients obtain and maintain safe, stable housing while coordinating Medicaid-covered services and community-based support that address medical, behavioral health, and social needs. This includes, but is not limited to, the following:

    • Provide a safe, non-judgmental, and empowering environment to clients.
    • Deliver trauma-informed, survivor-centered support, including appropriate response to domestic

    violence and crisis situations.

    • Uphold confidentiality, ethical standards, and organizational policies.

    Specific Duties:

    • Support Medicaid eligibility readiness and documentation gathering.
    • Deliver case management services that assist with housing search, applications, documentation, landlord coordination, and move-in planning.
    • Coordinate with community partners to connect clients to services addressing medical, behavioral health, and social needs.
    • Provide tenancy support services, including lease education, tenant rights and responsibilities, and housing stability planning.
    • Advocate on client’s behalf to secure safe and stable housing, which may include accompanying clients to housing appointments, acting as a liaison with landlords and negotiating leases, as well as provide guidance with rent subsidies, deposits, and other assistance related to housing.
    • Complete and submit accurate service documentation to support Medicaid billing compliance, including service notes, and required needs assessment.
    • Collaborate with internal finance teams and MCO representatives to resolve billing issues
    • Other duties as assigned.
