    Description

    Position covers Monmouth and Ocean Counties; Full Time: 4 Day Work Week

    Job Duties include, but are not limited to:

    • Develop a thorough understanding of the services funded by the DDD and ensure these services are utilized appropriately.
    • Establish contact with the individual and family, and support system as assigned. Interview the assigned individual and any appropriate family members or guardians.
    • Schedule and facilitate planning team meetings
    • Complete person-centered planning tool and individual service plans. Review and compile assessments/evaluations, identify supports and facilitate completion of discovery tools.
    • Write and distribute the ISP to the identified service providers and regularly review the ISP.
    • Monitor and follow up to ensure delivery of quality services in full consideration of the individual’s rights and needs.
    • Conduct monthly contact by phone or in person, quarterly face to face meetings, and annual home visits with each individual as assigned.
    • Organize and maintain confidential case records and other supporting documents.
    • Ensure individuals are free from abuse and neglect, report suspected abuse or neglect, and provide follow-up as necessary.
    • Participate in staff meetings and interdisciplinary team meetings to provide feedback and support to consumers and staff.
    • Develop and maintain professional relationships with appropriate services and agencies.
    • Provide on-call coverage as assigned

    QUALIFICATIONS:

    • Individuals living with a disability are encouraged to apply
    • Must have a Bachelors Degree; a degree in Psychology, counseling, social work or human services is preferred.
    • Must have 1 year of documented experience either paid or volunteer, working with adults with intellectual or developmental disabilities
    • Advanced computer skills including typing, Microsoft Office Suite, and/or Google Apps
    • Excellent verbal, written, and organizational skills
    • Knowledge of the resources and services available to people with disabilities in Ocean and Monmouth counties in New Jersey
    • Must be able to meet with individuals in their homes and in the community
    • Must be able to drive throughout Monmouth and Ocean counties or have reliable transportation
