    Description

    The Boston Center for Independent Living is a frontline civil rights organization led by people with disabilities that advocates to eliminate discrimination, isolation, and segregation by providing advocacy, information and referral, peer support, skills training, PCA services, and transitional services in order to enhance the independence of people with disabilities.

    Position Overview: A full-time, benefited position, assisting people with disabilities to acquire services to maximize their independence and health. Provides coordination of long-term services and supports, working closely with service providers and consumers enrolled in the One Care program, an innovative and nationally recognized health program for people with disabilities.

    Essential Duties and Responsibilities:

    • Representing the long-term service needs of One Care enrollees, advocating for consumers and providing education on long term supports and services to the consumer and health plan.
    • Providing long term supports and services coordination that includes in-home/facility assessments and contributing to and/or evaluating the consumer’s individualized care plan and monitoring the plan at their direction.
    • As a member of the health plan care team, participating in initial and ongoing assessments of the health and functional status of consumers and developing the community-based component of a care plan necessary to improve or maintain the person’s independence, health, and functional status.
    • Arranging and coordinating the authorization and provision of appropriate community-based long-term supports, services, and resources.
    • Assisting consumers to access personnel care attendant (PCA) services as needed.
    • Monitoring the appropriate provision and functional outcomes of community long term supports and services, according to the care plan, as deemed appropriate by the care team.
    • Determining community-based alternatives to long-term care.
    • Assessing appropriateness for facility-based long care, if indicated.
    • Working with BCIL staff as needed in service coordination efforts.
    • Performing other duties as assigned by the BCIL supervisor.

    Qualifications:

    • A bachelor’s degree in social work or human services or at least two years of relevant experience working with people with disabilities.
    • Member of the population to be served
    • Commitment to an independent living philosophy is essential.
    • Completion of training that includes education on person-centered planning and person-centered direction.
    • Knowledge of the home and community-based service system and how to access and arrange for services or the ability to quickly learn it.
    • Experience in conducting needs assessments for long-term care needs and monitoring long-term supports and services delivery.
    • Cultural competence and the ability to provide informed advocacy.
    • Ability to write an Individualized Care Plan and communicate effectively in English across complicated service and support systems or ability to learn quickly.
    • An ability to positively interact and advocate with a complex Interdisciplinary Care Team while representing a consumer’s needs and goals.
    • Ability to travel within the Greater Boston area and access consumers’ residences (some of which may not be accessible).
    • Ability to use varied databases.
    • Ability to work on tight timelines.
    • Ability to communicate in a language used by BCIL consumers (Spanish, Haitian Creole, Cape Verdean Creole, Portuguese, Vietnamese) a plus.

    Equipment Operated:

    Computers, copier, scanner, Microsoft Office, printer, and other office equipment/programs.

    Working Environment:

    Office area is a shared workspace divided by cubicles. Verbal communication can be heard from other staff in adjoining cubicles. Position requires both in office and remote work including seeing consumers in person in their homes.

    Hours: 40 Hours – Full Time

    Salary: $46,000.00 annually, depending on skills and experience. Language differential available for those speaking languages identified by BCIL.
