    Description

    The Incident Management Coordinator is responsible for oversight of the safety and welfare concerns of the people we support, including allegations of neglect or abuse, financial exploitation and errors in care provided. The Incident Management Coordinator acts as liaison with OPWDD and the Justice Center for incidents, investigations and necessary follow-up. The role supports the promotion of evidence-based practices and service feedback to inform on agency-wide regulatory compliance and person-centered service delivery.

    Essential Job Functions

    • Create and implement systems-driven information management for the agency's incidents and allegations and recommend system changes to enhance program efficiency and performance.
    • Oversee the inputting, maintaining and monitoring of incident reporting data management and documentation as per Parts 624 & 625 in tandem with Incident Management Unit (IMU), IRMA, Justice Center, and WSIR online tools.
    • Oversight of agency investigators (employees and consultants).
    • Oversight of the assignment and completion of investigations involving the health, safety and quality of life of people supported; and as needed, investigations of incidents relating to Corporate Compliance, Code of Conduct and HIPAA laws and regulations.
    • Act as the agency's liaison to the New York State Justice Center and OPWDD.
    • Oversee and ensure compliance and the timely maintenance of IRMA (Incident Report and Management Application) RIA (Restrictive Intervention Application) application within IRMA, and the Justice Center WSIR (Web Submission of Incident Report).
    • Assign immediate protections as needed.
    • Input and monitor investigatory updates/uploads into the state incident management system and communicate them to the state agency.
    • Distribute all incident CAPS, ensuring that all documented evidence of completion is received and meets the recommendation. Ensure follow through to finalization.
    • Update databases and complete corporate risk and incident information management.
    • Data compilation, review, analysis and reporting for routine and special projects/initiatives.
    • Demonstrate commitment and alignment with the Agency's mission, vision, and values.
    • Participation in assigned projects and committees.
    • Chair the agency Incident Review Committee.
    • Represent the agency with external committees as required.
    • Follow federal, state and local governmental regulatory guidelines pertaining to a safe, healthy and clean work environment, reporting health and safety concerns to the supervisor.
    • Identify and report any substantial Compliance Risks to the CCO or CEO as appropriate.
    • Other duties as assigned.

    Qualifications

    • Associate's degree required, (human or social services preferred).
    • Minimum of 3 years of compliance/quality improvement, utilization or program review, and/or administrative experience.
    • Knowledge of state and federal regulations as they pertain to Health Care Providers and operators of OPWDD services and the New York State Justice Center.
    • Minimum of 3 years of experience with OPWDD systems, including WSIR and IRMA.
    • Strong written and verbal communication skills
    • Strong organizational and time management skills.
    • Computer skills and capacity to navigate and enhance proficiency in software systems as needed.
    • Working knowledge of Microsoft Office and similar programs.

    Salary - $69,000 - $75,000.

    Compensation will be commensurate with job qualifications and work experience.
