About Us:
Community Counseling & Mediation (CCM) is a Brooklyn-based nonprofit organization committed to addressing the underlying causes of mental health and substance use challenges. We provide a broad range of culturally responsive social support and healthcare services to underserved communities across Brooklyn and Manhattan. Through programs like ASAP, CCM works to increase access to vital medical, behavioral health, and case management services for individuals affected by alcohol and substance use.
Position Overview:
We are seeking a compassionate and experienced CASAC or CASAC-T Substance Abuse Counselor to join our dedicated clinical team. The counselor will be responsible for conducting intake assessments, providing individual and group therapy, and supporting clients through their recovery journey. The ideal candidate is committed to evidence-based care and brings a client-centered, trauma-informed approach to treatment.
Key Responsibilities: Client Counseling & Treatment Planning
- Provide individual counseling to adults with substance use and/or alcohol use histories
- Facilitate 2–3 group counseling sessions weekly as part of the continuum of care
- Develop individualized, client-centered treatment plans focused on recovery goals
- Address addictive behaviors that impair clients’ health, judgment, and functioning
- Offer crisis intervention and support as needed
Assessment & Documentation
- Conduct comprehensive intake interviews and bio-psychosocial assessments
- Identify clients' needs, strengths, and barriers to inform appropriate referrals
- Maintain timely and accurate documentation in compliance with OASAS regulations
- Manage a caseload of approximately 25–30 clients, ensuring up-to-date progress notes, treatment plans, and discharge summaries
Team Collaboration & Case Management
- Participate in case conferences, staff meetings, and interdisciplinary team discussions
- Monitor client progress and adjust care plans as needed
- Coordinate referrals for medical, mental health, housing, and other support services
- Arrange home health care or aftercare planning when necessary
Minimum Qualifications:
- CASAC certification (Required)
- Associate degree required; Bachelor’s degree in Social Work, Psychology, or Sociology preferred
- Minimum of 2 years of experience working with individuals in recovery, preferably in an OASAS-regulated outpatient program
- Familiarity with substance use disorder treatment modalities and crisis management
- Strong interpersonal, communication, and organizational skills
- Sensitivity to the complex and diverse needs of clients in recovery
- Professional, ethical, and team-oriented
