    Job Title: Supportive Housing Case Manager

    About Us:

    Community Counseling & Mediation (CCM) is committed to supporting vulnerable populations by providing comprehensive behavioral health, housing, and social services. We aim to empower individuals experiencing homelessness and related challenges to achieve stability and self-sufficiency.

    Position Overview:

    The Permanent Supportive Housing (PSH) Case Manager will work closely with chronically homeless adults living in a supportive housing environment. This role supports CCM’s mission by coordinating services that promote empowerment, housing retention, and overall well-being. The Case Manager will collaborate with residents, housing management, and community partners to identify client needs and connect them with resources such as mental health and substance abuse referrals, employment and financial counseling, and healthcare services.

    Key Responsibilities:

    Client Intake & Assessment

    • Conduct comprehensive assessments covering housing, mental health, substance use, medical, financial, and employment needs.

    Individualized Service Planning

    • Develop and implement personalized service plans with clients to support their goals and housing stability.

    Housing Search & Placement

    • Assist clients with locating permanent housing, managing applications, liaising with landlords, and facilitating move-in processes.

    Ongoing Case Management

    • Provide consistent support through regular client check-ins, monitoring housing stability, addressing barriers, and linking clients to community resources.

    Crisis Intervention

    • Respond promptly to crises threatening housing retention, offering emergency support and coordinating with relevant agencies.

    Collaboration & Coordination

    • Partner with mental health providers, substance abuse counselors, healthcare professionals, and other service agencies to ensure holistic care.

    Advocacy & Outreach

    • Advocate on behalf of clients with landlords, government entities, and community organizations to promote housing stability and access to services.

    Documentation & Reporting

    • Maintain accurate and detailed client records, progress notes, and reports in compliance with program requirements.

    Qualifications:

    • Master’s degree required
    • Bilingual in English and Spanish required
    • Proven experience working with individuals experiencing homelessness, especially those with complex needs such as mental illness or substance use disorders
    • Strong knowledge of community resources and social service systems
    • Excellent case management skills including assessment, individualized planning, and crisis intervention
    • Outstanding communication and interpersonal skills to build trust with clients and collaborate effectively with stakeholders
    • Ability to work both independently and as a collaborative team member
    • Proficient in data entry and maintaining accurate records
    • Experience working with diverse populations preferred
