    Brandywine Valley Active Aging builds on a history of more than 40 years of continued service by the former Coatesville Area and Downingtown Area Senior Centers. While services are open to anyone who seeks to improve the quality of their life, regardless of their age, Brandywine Valley Active Aging’s programs and services are designed to offer the most value to individuals 50 and greater.

    GENERAL JOB DESCRIPTION: The Housing Case Manager is an experienced, social services professional responsible for providing high-quality, comprehensive, and intensive case management services to clients experiencing homelessness and ongoing support to those unstably housed. The role of the Housing Case Manager is to build trusting relationships with program clientswhich will enable the clientsto obtain and maintain housing. The Housing Case Manager is dedicated to supporting and empowering clients utilizing evidence-based practices, including a Housing First philosophy, Motivational Interviewing, Harm Reduction, and Trauma Informed Care.

    ESSENTIAL FUNCTIONS:

    Given the nature of the work, the position requires the ability to manage time effectively and prioritize work with little to no supervision.

    1. Key Responsibility (30% of time):Provide case management services to homeless and unstably housed clients in the over 60 age range, throughout the Coatesville and Downingtown area, so that clients obtain and maintain housing.
    2. Key Responsibility (20% of time): Build rapportand maintain positiveprofessional relationships with clients, landlords, service providers, and other community partners.
    3. Key Responsibility (30% of time):Accurately recordand maintain documentation, files, and data collection as required by local, state, and federal grants.
    4. Key Responsibility (20% of time):Work within the BVAA I&A team to ensure continuity of care, this includes attendingteam meetings, county-wide meetings/training and frequent communication with the Chester County Homeless Case Management System.

    MINIMUM QUALIFICATIONS (Knowledge, Skills, and Abilities):

    1. Possession of a high school diploma or its equivalent is required. Bachelor’s degree is preferred; however, comparable education and experience will be considered.
    2. Demonstrated knowledge and experience with area services,community partners, and resources.
    3. Desire to work with diverse populations and the ability to put service of others before self.
    4. Excellent interpersonal, verbal, and written communication skills.
    5. Ability to work independently and as a member of a team.
    6. Ability to prioritize work and to appropriately delegate to meet deadlines.
    7. Ability to maintain a high level of confidentiality, tact, and professionalism.
    8. Ability to function effectively within a crisis environment and make appropriate and safe decisions using a integrity-focused approach to providing interventions.
    9. In-depth familiarity with computers and computer software including Microsoft Office. Strong attention to detail and solid understanding of basic math skills.
    10. High level of accuracy, efficiency, and punctuality.
    11. Personal transportation and the ability to travel daily, if needed, throughout the County and occasionally for local, regional, or national conferences or training.

    WORK ENVIRONMENT AND PHYSICAL DEMANDS:

    While the work in this position is largely indoors, travel throughout Chester County does happen. When indoors, the work environment is protected from weather conditions but subject to some temperature fluctuations. Periodically during the year, the position is involved in outdoor activities and subject to temperatures below 60 degrees F and above 80 degrees F. Outdoor elements such as precipitation and wind are possible. On occasion, the environment may be noisy and distracting.

    The physical demands described are representative of those that must be met by an employee to successfully perform the essential functions of this position. Reasonable accommodations may be made to enable individuals who are differently abled to perform the functions.

    • Communicating with others to exchange information, instructions, and ideas by means of spoken words, typed or handwritten notes, forms and other work materials, use of a computer, laptop, or other electronic device.
    • Excellent hearing and vision for emergency situations.
    • Office work that primarily involves sitting and standing for sustained periods of time.
    • Ability to lift, adjust, push, pull, carry, slide, and move objects weighing 40+ pounds repeatedly and frequently in all directions.
    • Ability to bend, stretch, twist, squat, and reach in all directions for extended periods of time, up to 8-hours per day.
    • Other movements required from time to time may include, but are not limited to, stooping, kneeling, stepping, crouching, crawling, reaching, pushing, pulling, grasping, and repetitive motions hands, fingers, and wrists.
