    Position Summary

    The Case Manager provides compassionate and client-centered support to older adults and adults with disabilities living on limited incomes. The role helps enhance their quality of life by connecting them with resources, advocating on their behalf, and supporting them in accessing services that promote stability, well-being, and independence. Depending on assignment, the Case Manager may work with Pike Market Senior Center members and/or residents in PDA-owned Pike Place Market apartment buildings.

    Essential Job Duties

    Direct Support & Advocacy

    • Maintain a caseload of older adult clients with complex and intersecting needs—including chronic health conditions, cognitive changes, behavioral health concerns, mobility limitations, social isolation, financial insecurity, and experiences of homelessness or risk of losing housing.
    • Provide trauma-informed, client-centered case management that includes regular check-ins, crisis support, advocacy, navigation of homelessness and housing systems, and coordination with medical, behavioral health, and social service providers to promote long-term stability and quality of life.
    • Conduct needs assessments; develop and maintain individualized service plans.
    • Build trusting, supportive relationships with clients through active listening and collaborative problem solving.
    • Apply de-escalation and conflict resolution techniques to support clients in crisis or high-stress situations.

    Resource Navigation & Coordination

    • Assist clients in searching for and maintaining stable housing, including help with applications, follow-up, advocacy with housing providers, and connecting clients with resources that reduce barriers.
    • Support clients in accessing public benefits such as Medicaid, Medicare, SNAP, and Social Security.
    • Provide information, referrals, and coordination for medical, behavioral health, substance-use, and legal-related services.
    • Coordinate supportive services such as transportation, meal delivery, chore services, and other programs that enhance well-being.
    • Depending on assignment, make scheduled visits to clients in designated buildings, collaborating with building managers and other service providers.
    • Support clients with transitions, including hospital discharges, temporary housing placements, and moves to long-term care when appropriate.
    • Offer Information & Assistance to Senior Center members and the broader community.

    Teamwork, Communication, and Collaboration

    • Work cooperatively with Pike Market Senior Center & Food Bank staff, building staff, community partners, and other service providers to ensure coordinated care and seamless service delivery which sometimes means supporting different departments within the agency.
    • Communicate regularly with the Social Services Program Manager regarding progress, challenges, and emerging needs.
    • Share anonymized “wins” and success stories with the manager and team to help track program impact, support grant writing and funding opportunities, and highlight meaningful work that may otherwise remain unseen due to confidentiality.
    • Discuss emerging needs, barriers, and struggles in casework so the team can collaborate on strategies, identify resources, and support one another in addressing complex challenges.

    Schedule, Availability, and Professional Boundaries

    • This is a full-time position, 35 hours per week, typically Monday through Friday.
    • Maintain a balance between flexibility and predictability by adapting your schedule when needed to meet client needs—such as coordinating home visits, attending time-sensitive appointments, or responding to urgent circumstances—while also maintaining a regular and reliable work schedule that clients, colleagues, and partners can depend on.
    • Communicate clearly about scheduling, boundaries, and availability to support teamwork, client trust, and program consistency.

    Administrative Responsibilities

    • Maintain accurate case notes and input client information into Salesforce database, and in paper files, ensuring all records are complete, organized, and up-to-date.
    • Assist with data collection for reports, invoices, and grant requirements.
    • Contribute information for grant applications and other program documentation as needed.
    • Participate in individual supervision and team meetings
    • Participate in meetings and training.
    • Perform other duties as directed in support of the agency’s mission.

    Work Environment

    Primarily indoors at Pike Market Senior Center with occasional local travel to client homes, social service agencies, and medical facilities.

    Physical Demands

    • Prolonged periods of sitting and computer work.
    • Ability to lift up to 15 pounds occasionally.

    Desired Qualifications

    • Bachelor’s or Associate degree in social work, psychology, social services, behavioral sciences or related field; or combination of related education and/or work experience.
    • Demonstrated experience working with older adults, adults with disabilities, individuals with mental health and/or substance use disorders, individuals experiencing homelessness, or individuals facing housing or economic instability.
    • Strong verbal and written communication skills, advocacy, and de-escalation skills.
    • Experience working with confidential information and files along with maintaining client confidentiality.
    • Ability to prioritize and manage multiple tasks in a fast-paced environment.
    • Familiarity with Seattle-area service providers and community resources for low-income residents.
    • Ability to maintain professional boundaries and practice effective self-care.
    • Proficiency with Microsoft Office and comfort with email and online research.
    • Experience with client database systems such as Salesforce is a plus.
    • Bilingual skills—especially Spanish, Tagalog, or other Filipino dialects—are highly valued.
