The Wanda Alston Foundation (WAF) provides housing, support, and pathways to economic mobility to DC’s at-risk and homeless LGBTQ+ youth. Founded in 2008 to honor community organizer Wanda Renita Alston, WAF provides gender-affirming transitional housing, trauma-informed case management, life skills training, employment and education support, and a culturally safe environment for healing and growth.
Job Description
The Case Manager will provide youth-centered, trauma-informed case management services to LGBTQ+ young people ages 18 to 24 residing in the Foundation’s transitional housing program. This role delivers consistent, high-quality direct services in a residential setting, including supportive guidance, crisis intervention, and resource coordination to promote stability, well-being, growth, and long-term independence. The Case Manager will facilitate activities, manage conflict, provide on-site support, and connect residents to essential services. This is a primarily on-site position requiring in-person presence at the housing facility in Congress Heights, DC.
General Responsibilities
- Conduct comprehensive intake assessments and develop individualized service plans for each client, with a focus on achieving independent living outcomes.
- Provide weekly in-person case management services for residents, including facilitating groups, co-organizing events and workshops, and mediating roommate conflicts.
- Support clients in addressing barriers to stability, including food insecurity, transportation, financial challenges, non-clinical mental health concerns, substance use recovery, and legal or justice-related issues.
- Coordinate with the internal team to support residents in securing employment, setting education goals, and developing housing exit plans.
- Conduct crisis and risk assessment and provide appropriate interventions, with support from the supervisor.
- Develop and maintain community partnerships to streamline referrals and strengthen program offerings.
- Maintain accurate, timely, and thorough case notes. Track services, referrals, and outcomes in required databases. Additionally, support reporting for funders, audits, and program evaluation.
- Uphold the highest standards of confidentiality and integrity when handling client information, in accordance with HIPAA and organizational policies.
- Perform additional related duties as assigned.
Desired Skills and Qualifications:
- Bachelor’s Degree in Social Work, Psychology, Sociology, Counseling, or other related human services field, or four years of related work experience.
- Minimum of two years of experience providing case management services, supporting access to public benefits, working with individuals experiencing substance use challenges, or other related experience.
- Experience working closely with LGBTQ+ young adults.
- Knowledge of substance abuse/recovery, family systems, and medical and mental health treatment.
- Self-starter who can multitask and efficiently prioritize tasks.
- Demonstrated sensitivity to the needs and experiences of young adults who are experiencing homelessness, at risk of homelessness, or who have experienced trauma.
- Must be able to exercise patience, professionalism, and understanding even in escalated or crisis situations.
- Bilingual proficiency (English/Spanish) is strongly preferred.
Physical Requirements:
This job involves a mix of sedentary and active duties. While performing the duties of this position, the employee is regularly required to talk or hear. The employee is frequently required to use their hands or fingers and utilize a computer or laptop to complete their duties successfully. The employee may occasionally be required to lift packages up to 50 lbs. The employee is occasionally required to sit for prolonged periods. Specific vision abilities required by this position include close vision, distance vision, and the ability to adjust focus. The noise level in this work environment is usually low to moderate. The employee may occasionally be required to use their hands and fingers to perform janitorial and light cleaning duties as well as assist maintenance with flipping housing units.
Other Terms of Employment:
- Candidates must complete and successfully pass a drug test, alcohol test, TB test, sex offender registry check, Child Protection Register check, and criminal background check prior to employment and periodically thereafter.
- $60,000 - $70,000 annual salary.
