    About Riverstone:

    Riverstone has been providing services to older adults in Northern Manhattan since 1985 with a commitment to help older adults to remain active and living safely in their home and community for as long as they choose. Many of the older adults who seek support at Riverstone live below the poverty line and have limited or no proficiency in English. Our bilingual staff are culturally and linguistically experienced in providing highly competent and compassionate service. The organization operates one older adult center, a comprehensive social services division, and a Memory Center - a social adult day program for individuals of any age with memory loss due to Alzheimer’s or other form of dementia.

    Level of Language Proficiency

    Qualifications:

    Master or Arts, preferred, and Bachelor’s Degree considered, in the Human Services filed, preferably aging, with demonstrable varied years of experience are a requisite. Experience running programs, working with clients and supervising staff roles and are required; works well with people, detail oriented and well organized, works well under pressure, can prioritize, highly motivated, can work independently, shows initiative and has leadership qualities. Must be able to communicate well in both English & Spanish.

    Responsibilities:

    Administrative:

    • Work with the Director to sustain and uphold the mission in order to cultivate a safe and productive environment in the Adult Day Care Program and Services.
    • Direct supervision of program staff, interns, and consultants.
    • Coordinate with the administrative, Care Manager, and program staff to ensure that families receive all necessary help with entitlement applications and other services, which they may need.
    • Coordinates all program activities involved with the safety, compliance, and effective operation of the Memory Center.
    • Responsible of participant care plans in coordination with the appropriate program staff.
    • In conjunction with the Administrative Assistant coordinate and maintain client’s records, which include, filing, and data entry, maintain statistics as required (SOFA), program attendance, transportation, monthly inventory, and case notes.
    • Together with the Case Manager provides intakes and follow-up referrals to the program.

    Program:

    • Plan, prepare, and facilitate recreational activities which include: Arts & Craft, Music, Cognitive, & Physical Exercise, Group Discussions, etc.
    • Coordinate work of the Program Leaders, Program Assistants, and Program Volunteers.
    • Provide training and guidance for all program staff that will provide helpful skills as well as any necessary training needed to be in accordance with the DOH/ State/City requirements.

    General:

    • Community outreach and education seminars regarding Alzheimer’s disease and related disorders.
    • Lead support group for Family/Caregivers, when necessary.
    • Document any incident(s), where client(s) or staff may be in an inappropriate or dangerous situation.
    • Document all contact with client and their caregivers.
    • Coordinate programming in the absence of the Memory Center Director under the Supervision of the Executive Director.
    • Other tasks related to the successful functioning of the Memory Center, as determined by the Director.

    Compensation: $65K - $70K, and commensurate with experience
