    Per Diem Diabetes Educator Job Description

    About the Diabetes Foundation, Inc.: Since 1990, the Diabetes Foundation (DF) has been dedicated to providing individuals living with diabetes, access to high-quality, affordable, and accessible resources, to help them manage their care, from medical assistance, to free education and coaching, to a wealth of personalized resources. We serve thousands of individuals each year helping the provide help, hope and better help to children and adults throughout the state of New Jersey.

    Reports Directly To: Senior Manager: Patient Education and Support Services

    About the Diabetes Educator Role: The Diabetes Foundation is dedicated to helping children and adults who are at risk of developing diabetes to those who are living with Type 1, Type 2, and gestational diabetes. You will work directly with our participants to educate them about the important aspects of self-care related to diabetes management from physical activity and healthy eating, to monitoring and medication management. You will work with a small team of our staff and educators to deliver education to communities and individuals throughout the state of New Jersey – both in person and virtually. The hours vary up to 10 hours weekly.

    Primary Duties and Responsibilities:

    • Deliver diabetes education to individuals with diabetes or at risk for diabetes in a group setting or individually (virtually or in-person) within the state of New Jersey.
    • Educate using our ADCES accredited DSMES curriculum and materials which focus on the ADCES-7.
    • Collect self-assessments and support participants in goal setting and behavior modification throughout the program.
    • Document education visits and perform follow-up calls to participants as needed.
      1. Participate in reporting data to the Manager as needed.

    May perform MNT intervention and education (in person or virtually) if person is a registered dietitian.

    • Document visit, set progress plan and provide appropriate interventions for patients.

    Promote diabetes-related best practices within public spheres to boost well-being.

    May be asked to collaborate with other educators and support our other education classes.

    Able to attend quarterly meetings

    Able to participate in bimonthly or monthly meetings with senior manager

    • As time allots, support DF through development of webinars and additional education material to support the public
    • Record educational diabetes self-management videos to be distributed amongst participants, referrers and caretakers in a professional studio

    Core Competencies:

    • Licensed Dietitian, Nurse or Pharmacist
    • Certified Diabetes Educator preferred
    • Spanish Speaking and/or French Creole Speaking a plus
    • Dedicated to mission of Diabetes Foundation
    • Diligent in fulfilling responsibilities, self-motivated
    • Adept at being flexible, and able to navigate challenges
    • Flexibility to work evenings and weekends as needed
    • Strong interpersonal, written and oral communications skills
