    Description

    Duties and Responsibilities:

    • Provide an assessment of the client’s mental health and substance abuse needs to develop a service plan.
    • Provide supportive counseling and advocacy for clients suffering from co-occurring disorders, including substance abuse.
    • Provide DSM-5 diagnoses and the ability to utilize research-based treatment modalities effectively.
    • Understanding and practicing professional ethics code of conduct and HIPAA compliance.
    • Maintain up-to-date client records and statistical data.
    • Prepare documentation for the client’s file (intake, encounter notes, treatment plan, etc.)
    • Maintain timely and accurate clinical documentation.
    • Report counseling/case management services to the supervisor weekly.
    • Refer clients for an appointment with a psychiatrist as needed.
    • Provide referrals for community resources and social services programs within Hamdard’s service tracks.
    • Attend staff meetings and other Hamdard Center activities.
    • Participate in staff training, development, and support activities.
    • Develop and implement new processes and procedures for services provided.
    • Travel to agency locations in Chicago Northside and Addison Township required
    • Other duties as assigned that will support the mission of Hamdard.

    Qualifications:

    • Masters degree in social work or counseling with substance abuse experience
    • 1 year experience providing behavioral health services to adult and adolescent clients.
    • Experience with MAT based services preferred
    • High level of comfort working within the medical model on a team of primary care providers while using a consultative style.
    • Ability to exercise sound judgment in managing patient crises calmly, consistently, and equitably.
    • Ability to manage a large case-load of clients presenting with complex needs within a community health setting.
    • Bilingual preferred
    • Current Illinois license as a behavioral health professional required (LCSW or LCPC)
    • CADC licensure preferred
    • Excellent organizational skills are required.
    • Compliance with organizational TB, and Influenza vaccination policies, required
