    Tri City Health Partnership is a dedicated, non-profit organization committed to providing medical and dental care to underserved people who live or work in Central Kane County. Our mission is to promote a healthy lifestyle in an environment of mutual respect, and everything we provide for TCHP patients is completely free to the patient. TCHP will be celebrating our 25th anniversary in 2026.

    Purpose of Role:

    As the Grant Writer for Tri City Health Partnership (TCHP), you are responsible for securing funding that ensures the sustainability and growth of our free and charitable medical and dental services. Your work helps translate our mission into measurable support by developing compelling proposals, managing grant reporting, and identifying new funding opportunities.

    Core Responsibilities:

    • Grant Writing & Proposal Development
      • Draft, revise, and submit persuasive and accurate grant proposals for foundations and corporations.
      • Tailor each proposal to the funder’s guidelines while clearly conveying TCHP’s mission, impact, and funding needs.
      • Ensure that all applications are complete, accurate, submitted on time, and align with organizational goals.
    • Grant Management & Reporting
      • Maintain an up-to-date calendar of grant deadlines, including internal deadlines and application and reporting timelines.
      • Prepare and submit timely and accurate narrative, outcome and financial reports as required by funders.
      • Track outcomes, outputs, and success stories in coordination with the fundraising team and clinic staff.
    • Research & Prospecting
      • Proactively identify new funding opportunities that align with TCHP’s mission and programs.
      • Maintain a pipeline of potential funders, with notes on deadlines, priorities, and fit.
      • Monitor shifts in funding landscapes to stay ahead of new opportunities or changes in funder priorities.
    • Collaboration & Communication
      • Work closely with the Resource Development Manager, Executive Director and program staff to gather data, patient stories, and updates needed for grant submissions.
      • Help maintain consistent, mission-driven messaging across all grant communications.
      • Support donor communication, acknowledgments, and additional development writing as needed.

    Required Qualifications:

    • At least 1-year experience in grant writing, reporting, and project management
    • Knowledge of grant/development best practices
    • Proven experience managing relationships with grant funders
    • At least 1-year experience with a fundraising database software and a grant management software, preferably DonorPerfect and Instrumentl

    Salary and Benefits:

    • Salary will be based on relevant experience and qualifications. Salary range for this position is $50,000 to $62,500/year.
