Purpose: CHCN exists to provide accessible, community-centred primary health care and supportive services to individuals and families - especially those facing barriers such as lack of a family doctor, socioeconomic challenges, isolation, or complex health needs - and to strengthen community health through education, prevention, and partnership.
Programs and Services
The Community Health Centres of Northumberland (CHCN) provides a comprehensive range of integrated primary health care services and community-based programs that support the physical, mental, and social well-being of individuals and families across Northumberland County. Services are delivered by an interdisciplinary team including physicians, nurse practitioners, nurses, social workers, dietitians, diabetes educators, dental professionals, counsellors, health promoters, and community outreach staff.
Primary Health Care
• Comprehensive primary care delivered by family physicians and nurse practitioners, including assessment, diagnosis, treatment, chronic disease management, preventive care, and referrals.
• Urgent day clinic services for non-life-threatening health concerns.
Chronic Disease Management & Specialized Care
• Diabetes Education Program providing individualized and group-based education, lifestyle counselling, and self-management support.
• Geriatric Assessment & Intervention Network (GAIN) supporting frail seniors with complex needs through coordinated assessment and care planning.
• Memory Clinic (MINT) offering assessment, education, and support for individuals experiencing cognitive changes.
Mental Health & Addictions Services
• Individual and group counselling, mental health supports, and addiction services delivered within an integrated, team-based care model.
Dental and Oral Health Services
• Subsidized dental care, including preventive and restorative services, for individuals with limited or no dental insurance.
Nutrition, Wellness & Food Security Programs
• Nutrition counselling, healthy eating education, and lifestyle programs.
• Food security initiatives including emergency food supports, community food markets, community gardens, and shared meal programs that promote access to nutritious food and reduce social isolation.
• Group wellness programs focused on chronic pain, arthritis, physical activity, mindfulness, smoking cessation, and caregiver support.
Rural Outreach Clinics & Mobile Services
• Primary care, chronic disease support, health promotion, and system navigation delivered through rural outreach clinics and mobile services, improving access for residents in underserved and geographically isolated communities, including outreach to Alderville First Nation and surrounding rural areas.
• Services are provided in partnership with local organizations to reduce transportation barriers and improve continuity of care.
Youth Health & Youth Wellness Hub Services
• Youth-focused health and wellness services using a Youth Wellness Hub model, offering low-barrier, integrated supports for young people.
• Services include mental health counselling, primary care access, health education, and connections to community and social supports, with an emphasis on early intervention and prevention.
