The Client Engagement & Post-Discharge Support Volunteer helps create a welcoming and supportive environment for individuals receiving Behavioral Health services. Volunteers play a crucial role in improving the client experience by greeting clients in the waiting area, offering assistance when needed, and helping maintain a positive, calming atmosphere. Additionally, volunteers provide follow-up support to recently discharged clients through structured post-discharge phone calls that reinforce continuity of care and encourage ongoing engagement.
- Warmly greet clients as they arrive and wait for services, helping ensure they feel welcomed and supported.
- Engage clients in a professional, compassionate manner while they wait, offering assistance or notifying BH staff as needed.
- Help maintain a calm, organized waiting room environment.
- Make post-discharge outreach calls using the agency’s approved scripts.
- Confirm clients understand their discharge plan and provide reminders about community resources or next steps.
- Document all call outcomes per agency guidelines and report concerns to designated staff.
- Must have strong written and computer skills
- Uphold confidentiality and adhere to all HIPAA standards during client interactions.
- Demonstrate professionalism, empathy, and strong customer-service skills at all times.
- Bi-lingual preferred (but not required)
