We don't focus on the finality of death; we focus on the practical side of living. Our job is to assist our patients and their families through the physical, emotional and spiritual challenges of life's final transition.
About Us
Transitions Care LLC is a premier, multi-state, Midwest hospice care service. After a less than favorable experience with hospice when his father was terminally ill, our founder felt compelled to create an environment that met and exceeded the fundamental expectations his experience had not. Our founder believed the focus of hospice should be on living, not dying, and it is the obligation of a hospice to be available 24 hours a day, 365 days a year. He has created an environment and team that believes and lives by his commandments: TO COMMIT TO HOSPICE, EACH PERSON IS UNIQUE, AND ALWAYS DOES THE RIGHT THING.
