Heartland Hospice offers support, care, and comfort to the terminally ill and their families. If a individual has been diagnosed with a terminal illness, and can no longer benefit from a curative treatment program (life expectancy of 6 months or less), Heartland Hospice can help. The hospice staff is actively involved in all aspects of the patient's care. This professional staff collaborates with the patient, family, attending physician, and staff (if patient is residing in an extended care facility. The hospice team concentrates on providing comfort, pain and symptom management, spiritual and emotional support. We believe that a good relationship with the patient's health care providers is essential to the emotional and physical well-being of our patients and their families. We develop an individualized care plan which we monitor and update regularly, adjusting it to meet the patient's needs. Both patients and family members are included in this decision-making process.
