Interim HealthCare of Northwest Pennsylvania (Interim Hospice Erie PA/Meadville PA)

Interim Hospice started serving patients in Erie, PA in 2019. In 2020, we established outreach in Meadville, PA providing hospice services in both Erie and Crawford counties. In 2019, our agency received a "Zero Deficiency Survey" from the PA Department of Health. We obtained accreditation with CHAPS (Community Health Accreditation Partner) in 2020, and maintain active compliance with the CHAPS Standard of Excellence. Currently, we are licensed to provide hospice care in five counties including Erie, Crawford, Mercer, Warren and McKean. Our team consists of registered nurses, health aides, Physician Medical Director, social worker, spiritual care counselor and volunteers in training. Our agency bereavement program provides support and ongoing connection with family members for a period of thirteen months following the passing of a loved one. Our goal remains to be that hospice known in our communities for creating a space that allows the patient and family to find peace of mind, body and spirit during one of the most beautifully complicated times of their lives.
