Who We Are: 

Empower Nation is a direct support organization providing behavioral health services to eligible members. Empower Nation’s mission is to support access and eliminate barriers by carefully coordinating a network of resources to dramatically reduce the rates of poverty by guiding individuals on a custom-made path starting with graduation through secondary education, into job placement and career advancement.  

Our goal is to provide each of our members with foundational knowledge and resources to help bridge the gaps to success. Using a strength-based approach, we recognize each individual’s potential for growth, and we support their journey from adversity to stability to success. Through the vision of empowerment theory, we help guide our members toward building autonomy and enhancing their capacity to shape their futures. 

Empower Nation will conduct weekly contact sessions with caseload members to provide understandable support and plans to members and to listen carefully to understand their needs. Licensed counselors will work diligently with the members to expand on their strengths and empower their capacity to succeed.  

Contact sessions will include:

• Evaluating members (existing, new and prospective) based on their needs, limitations and desires 
• Addressing patient concerns and goals while maintaining weekly communication with the member as well as a case manager 
• Building rapport with members and their families, if necessary 
• Coordinating care with Junior Success Agents to address needs and promote the overall well-being of member 
• Completing psychosocial intake assessments and ongoing clinical assessments 
• Developing collaborative treatment plans with members  
• Maintaining member confidence and keeping the information confidential. 
• Fostering members’ action or adjustment by interpreting attitudes and patterns of behavior, explaining and suggesting new options to empower members to progress towards their goal.
