Midwest Care Management Services

Midwest Care Management provides guardianship and care coordination to older adults with dementia and to people throughout the life cycle with disabilities. We are a safety-net for people who lack support systems, often preventing isolation, neglect, and exploitation. We assist with medical, residential, and financial management for our clients.
