    Our Patient Navigators are invaluable in their facilitation of healthcare services that focus on patient needs. As members of the QMA healthcare team, Patient Navigators have a caring spirit and superb knowledge of human services for navigating patients through the healthcare system. In support of patient advocacy, they work closely with patients to overcome any obstacles to continuing treatment. Patient Navigators collaborate and coordinate care with external human service providers including home care agencies, community agencies, mental health professionals, and insurance plans (private and governmental). In addition, our Patient Navigators will assist oncology patients with completing advance directives; ensure accurate documentation in EHR, inclusive of screenings, progress notes, and billing procedures.

    RESPONSIBILITIES AND DUTIES

    The Patient Navigator will work in collaboration with physicians, physician assistants, medical scribes, nurses, and all other clinical and administrative support staff to provide healthcare services for adult oncology patients. In this role, you will evaluate patients’ psychosocial needs including transportation, home care and meal deliveries.

    DESIRED SKILLS AND EXPERIENCE

    • To qualify, you must have an associates or bachelor’s degree in social work or related human services field.
    • Ideal candidates will have a minimum of two years of experience in the field of human services.
    • In addition, successful candidates will have excellent interpersonal skills, knowledge of insurance programs, and patient care skills.
    • Prefers Bilingual Spanish or Mandarin speaking.
