    ORGANIZATION: PHILIPSTOWN BEHAVIORAL HEALTH HUB (“The Hub”)

    LOCATION: New York State: Mid-Hudson Valley Region

    Putnam County (Philipstown / Cold Spring)

    SALARY: $50,000-$55,000, non-exempt

    WHO ARE WE?

    Known as “The Hub”, the Philipstown Behavioral Health Hub is a non-profit organization that provides local access to behavioral health and addiction resources in Putnam County, NY (Philipstown region).

    Mission Statement:

    The Hub connects our community to personalized mental health care including addiction and recovery resources, while educating, reducing stigma, and empowering individuals and families.

    Over the past decade, the region has lost many lives to fatal overdoses. Many more residents struggle with issues related to depression, anxiety, and addiction. Local residents have difficulty finding and accessing critical treatment and prevention resources, because most services are located outside the community.

    The Hub was created to give residents a place to turn for local access to these resources. The Hub provides a single point of entry to mental health and addiction programs, services, and education within the greater Philipstown area. All the services we offer are free.

    The Hub was started in 2019. It is now expanding its work in many ways, including more community outreach, engagement, and programming, as well as developing a model for other communities. These new opportunities require an enthusiastic and committed staff member to join our team!

    WHAT DO WE DO ?

    We coordinate resources and services to ensure that our residents get the help they need. We provide personalized and confidential support to participants and their families.

    We host events to educate the community, raise awareness, and reduce the stigma associated with issues of mental health and addiction.

    We plan and implement programming and support groups based on the current needs of members of the in community.

    We partner with community-based entities, such as local school districts, businesses, law enforcement, medical providers and non-profits to create innovative programming.

    WHO ARE YOU?

    You have a passion for helping people support themselves and/or their families through difficult times, and you are comfortable working with individuals from a variety of socioeconomic backgrounds. You understand how diversity shapes and influences the human experience. Specifically, you are interested in mental health issues and the challenges individuals facing addiction experience.

    You want to deliver high-quality behavioral health care to the community, and you are proud to participate in the effort to incorporate and sustain best practices within an organization.

    You enjoy work that challenges you on a daily basis and allows you to fully educate and engage both with participants and the broader community, including various organizations and institutions. You are responsible, organized, efficient, and willing to do various tasks. You are able to perform multiple assignments simultaneously, and you can work both independently and in a team setting.

    You welcome the opportunity to expand the capacity of an organization in delivering the best service to its clients through quality grant-funded programming and services, and you understand the need for evidence-based strategies to achieve positive outcomes.

    You have ability and interest in facilitating various support groups. You can coordinate a programming schedule and maintain communication with group facilitators.

    You are interested in developing programming for young people/teenagers.

    You have marketing and design skills and are familiar with Canva (or similar), Squarespace (website platform), Mailchimp (for digital newsletters) and event platforms such as Zeffy and Eventbrite.

    You are excited about running an organization’s social media (Facebook and Instagram) and you have creative ways to share news, events, campaigns, stories and successes, and you have good ideas for engaging people on social media.

    You are eager to engage with the community, whether it’s helping to build coalitions and partnerships, planning events, or attending county-wide social services meetings.

    You have some background or interest in learning about grant writing.

    You appreciate working with talented colleagues as passionately dedicated to compassionate excellence as you are. You have great communication skills (effective/active listening, verbal and written), and you have the ability to deal tactfully and diplomatically with other Hub employees, government officials, professional groups, participants and their families, and the general public.

    WHAT IS THE POSITION?

    The Community Connector & Care Coordinator (CCCC) reports to the Executive Director and works with other Care Coordinators. The CCCC provides comprehensive care coordination, individual and family support services, and connections to community and social supports through participant-centered care planning. The CCCC, under the supervision of the Executive Director, provides services consistent with the policies and procedures of the organization. This individual is sensitive to the fundamental components of recovery, in a person- and family-centered approach that incorporates cultural appropriateness. This individual ensures that the organization’s mission, goals, and objectives are accomplished by fulfilling job and team responsibilities, as assigned.

    • Engage participants and their support systems/providers through intake, ongoing relationship-building, goal setting, progress tracking, and follow-up in a confidential and compassionate manner.
    • Maintain knowledge of treatment options, referral sources, discharge planning, and after-care services to support participant needs effectively.
    • Enter and maintain accurate participant information, IntakeQ notes, metrics, reports, and case review documentation.
    • Collaborate with the care coordination team to build and maintain a comprehensive community resource network and strong provider relationships, including site visits as needed.
    • Ensure timely follow-through and continuity of care for all open participant cases until services are appropriately concluded.
    • Complete internal and external funding requests and assist with grant-related documentation and reporting as needed.
    • Collect data, maintain records, prepare reports, and provide metrics to support grant applications, newsletters, marketing, and organizational initiatives.
    • Provide consultation, collaboration, and direct support to fellow care coordinators, particularly in complex or urgent cases.
    • Support individuals and families during crises by coordinating communication, collaborating with relevant agencies, and helping develop personalized plans of care while maintaining privacy and confidentiality.
    • Communicate regularly with the Executive Director regarding active cases, crises, and challenging community situations.
    • Participate in case conferences, trainings, workshops, seminars, county meetings, and team collaboration sessions.
    • Build and engage with community coalitions aligned with the Hub’s mission.
    • Operate the “You Matter Line,” providing weekly check-in calls, emotional support, resources, and guidance to residents as part of an ongoing mental health support program (non-crisis line).
    • Initiate, execute, and sustain programs, activities, and events for participants and community members, including facilitating groups and developing ideas for new or improved services and programming.
    • Contribute recommendations regarding best practices, participant satisfaction, and client outcomes.
    • Create marketing and outreach materials, including brochures, flyers, graphics, newsletters, social media content, email campaigns, and monthly event calendars.
    • Maintain and update the organization’s website and social media platforms while distributing promotional materials to community organizations and institutions.
    • Research and generate materials relevant to participant care and organizational programming to support grant proposals and grant-writing efforts.

    WHAT ARE YOUR QUALIFICATIONS?

    • Bachelor’s degree in social work, psychology, or a related field required.
    • Minimum of two years of relevant experience, including community mental health, addictions/substance abuse, case management, or crisis intervention. Previous experience within Putnam, Dutchess, Orange, Rockland, or Westchester Counties preferred, but not required.
    • Familiarity and comfort working with individuals from diverse cultural and socioeconomic backgrounds.
    • Understanding of and sensitivity to the core components of recovery within a person- and family-centered approach that incorporates cultural responsiveness.
    • Valid driver’s license required.
    • Reliable access to a vehicle required.
    • Ability to work some evening and weekend hours and maintain a flexible schedule at least a couple of days each week.
    • Residence in southern New York State, northern New Jersey, or southern Connecticut preferred.
    • Lived experience with mental health issues or addiction/recovery (personally or through friends/family) preferred, but not required.
    • English required; Spanish preferred, but not required.
