Position Title: MAP Nurse
Department: DDS Residential Program
Reports To: Director of Services /Residential Services Director
Salary Range: $38 - 45 hr

Position Summary
The MAP Nurse (RN or LPN) is responsible for overseeing and coordinating the medical and healthcare needs of individuals residing in DDS residential programs. The RN ensures compliance with DDS, MassHealth, and agency regulations while promoting health, safety, independence, and quality of life. This role collaborates closely with interdisciplinary teams, guardians, families, primary care providers, specialists, and residential staff to ensure continuity of care and implementation of best nursing practices.
Qualifications
Required
- Bachelor’s degree in nursing (BSN preferred)
- Active Registered Nurse (RN) or LPN license in the Commonwealth of Massachusetts
- Current Medication Administration Program (MAP) certification required
- Minimum of two (2) years of experience working with individuals with developmental disabilities, mental health conditions, or in a residential/human services setting
- Knowledge of DDS regulations, MassHealth requirements, and Medication Administration Program (MAP) standards and have knowledge of DPH regulations.
- Valid driver’s license and reliable transportation

If you are not currently MAP certified, are you willing and able to obtain MAP certification within 90 days of hire?

Responsibilities
The responsibilities of the Registered Nurse include:
- Administer medications and treatments in accordance with physician orders, DDS regulations, and agency policies
- Conduct comprehensive nursing assessments and develop individualized Plans of Care
- Complete monthly nursing summaries and maintain accurate medical documentation
- Monitor individuals’ health status and promptly document and respond to any changes
- Maintain current physician orders and ensure compliance with all DDS, DPH and MassHealth documentation requirements
- Coordinate routine, urgent, and specialty medical appointments, including preparation and follow-up
- Communicate health status updates with guardians, families, PCPs, specialists, and other providers
- Provide training and education to residential staff on medical care, medication administration, infection control, and emergency procedures
- Ensure compliance with Medication Administration Program (MAP) standards and monitor staff competency
- Review referrals and provide clinical input for prospective admissions
- Advise leadership on medical or psychiatric concerns impacting individuals’ safety and well-being
- Collaborate with external providers and community resources to ensure access to quality healthcare services
- Assist with emergency preparedness planning, including public health emergencies and natural disasters
- Maintain RN licensure and complete required continuing education
- Stay current with best practices in developmental disabilities, mental health, and trauma-informed care
- Provides for ongoing Medication Administration Program (MAP) training that prepares employees for MAP certification exam and medication related work duties.
- Provides re-certification classes and evaluation for staff Schedules MAP tests and maintains a test database.
- Provides individual MAP tutoring.
- Works with the MAP consultant to establish workflow for all Victory Human Services staff attending the MAP program
- Tracks staff certification status, and maintains certification database and Bay Net list.
- Performs site assessments of MAP operations and directs staff in needed follow up activities to fulfill requisite MAP activities.
- Maintains MAP licenses for all housing sites; assists house staff in preparing for MAP, licensing, and accreditation inspections.
- Works with DMH, DPH, and American Red Cross to fulfill all MAP requirements.
- Perform other related duties as assigned
