Position Summary
Working under the direction of the VP of Community Building – Asian Communities, this community services specialist provides awareness, outreach, education, and engagement activities to underserved and high-risk communities, using best-practice, culturally sensitive approaches. This role conducts community-based activities and education sessions in English, Mandarin, and Cantonese on brain health, memory changes, and available services, and uses grassroots methods within cultural and underserved communities to identify awareness, training, and service needs and to address barriers such as trust, access, and language — working with community members, organizations, and providers serving older adults and families impacted by dementia within the Chinese-speaking community.
Primary Responsibilities
- Conduct information sessions on CaringKind services and supports at local health fairs, expos, and with healthcare providers and community-based organizations to increase awareness and access.
- Build relationships with healthcare providers, community-based organizations, special interest and faith communities, research institutes, and elected officials, and identify community-based satellite locations, to expand awareness of and access to services and education for the diverse range of Asian populations impacted by dementia.
- Cultivate relationships with referring partners in health systems — hospitals, nursing homes, diagnostic centers, research centers, and primary care physicians and their staff — to establish CaringKind as a vital extension of the care team.
- Help organize brain health, cultural, musical, art, and respite programs and celebrations to improve quality of life for families affected by dementia.
- Recruit volunteers and assist with walks and fundraising events.
- Support the VP with scheduling and follow-up for outreach activities and community partner meetings, and help manage volunteer mentor outreach and the Nursing Home Ambassador Initiative.
- Coordinate with CaringKind Navigators and other specialists to support individuals and families with care planning and engagement.
- Provide phone support, assist with wanderer safety/Love Tag applications, and deliver other direct support services.
- Collaborate and case-conference with other Community Builders and professional staff to share resources and best practices for culturally sensitive approaches, and align efforts with partner initiatives (e.g., Dementia Friends NY, ALTER, BOLD).
- Monitor activities against deliverables and provide regular communication with leadership on progress, challenges, and successes.
- Document all activities and client interactions in Salesforce CRM in a timely manner.
- Perform other duties as assigned, including dementia-related research projects.
Expectations and Required Skills
- Bachelor's degree in a related field (e.g., social work, community or public health, gerontology, nursing, human services), or a combination of education and relevant experience.
- Fluent public speaking ability in English, Mandarin, and Cantonese (required).
- 1–3 years of dementia/aging services experience preferred.
- General knowledge of Alzheimer's disease and related dementias
- Knowledge of community-based supports, programs, and aging network services.
- Ability to take direction and execute independently, while also working effectively as part of a team.
- Excellent written and verbal communication skills.
- Proficient with technology; familiarity with Office 365 and Salesforce preferred.
- Scheduling flexibility for monthly evenings and weekends required; willingness to travel.
