    ABOUT CARPENTER’S SHELTER:

    Since 1988, Carpenter’s Shelter has met the immediate needs of people experiencing homelessness in the City of Alexandria. Through the operation of emergency shelter, day shelter, and winter shelter as well as rapid rehousing and permanent supportive housing programs, Carpenter’s Shelter identifies housing solutions and supports sustainable independence for more than 900 individuals and families each year.

    TITLE: Permanent Supportive Housing Case Manager

    REPORTS TO: Therapeutic Services Director

    GENERAL DESCRIPTION:

    The Permanent Supportive Housing Case Manager provides individualized, housing-focused case management services to individuals residing in Permanent Supportive Housing, including The Bloom and scattered-site housing throughout the community. PSH residents may present with serious mental illness, substance use disorders, and/or other co-occurring issues which have impacted their ability to obtain/maintain permanent housing. The Permanent Supportive Housing Case Manager provides individualized services, tailored to each client, and general support related to budgeting, rent payment, connection to resources, maintaining a household, and other areas which support their basic needs and enable them to remain stably housed. The Permanent Supportive Housing Case Manager is responsible for collaborating with representatives from local government, other non-profit agencies, and the faith community.

    SPECIFIC RESPONSIBILITIES:

    • Provide specialized support to residents using a Housing First approach and a client-centered and trauma-informed lens to help residents achieve successful tenancy in permanent housing
    • Conduct intake assessments and create individualized service and housing stabilization plans.
    • Provide consistent, person-centered case management through in-home, in-office visits and in the field.
    • Develop and implement housing stabilization strategies to prevent lease violations, evictions, and returns to homelessness.
    • Conduct comprehensive strengths-based housing barrier assessments to collect financial, employment, housing, educational, mental health, substance abuse, etc. histories to support the development of short and long-term goals to support housing stability
    • Conduct monthly home visits to assess housing stability, unit condition, client well-being, and ongoing supportive service needs.
    • Conduct at least monthly case management meetings to assess client needs, review progress toward housing stabilization goals, and update service plans as appropriate.
    • Support residents in maintaining successful tenancy through budgeting, lease education, conflict resolution, rent and utility payment planning, and early intervention when housing stability concerns arise.
    • Assist residents with increasing income and accessing mainstream benefits, employment, education, or vocational resources based on individual goals.
    • Respond appropriately to housing crises, behavioral health concerns, and emergencies by coordinating with supervisors and community partners to maintain client safety and housing stability.
    • Ensure residents are connected to mainstream resources, and community benefits they are eligible for
    • Liaison between residents and property management regarding maintenance, grievances and tenant needs
    • Review resident ledger regularly to ensure no discrepancies
    • Coordinate with external partners (e.g., healthcare, behavioral health, financial coaches) to ensure wraparound supports.
    • Assist residents in developing and expanding their support systems; provide linkages to services and mainstream resources based on the household’s individualized needs
    • Assist clients with access to public benefits, legal documentation, and essential services.
    • Develop and maintain relationships with community resources and partners to efficiently support and advocate for households served
    • Complete accurate intakes by collecting all necessary information for program regulations
    • Maintain accurate, timely, and complete client files, case notes, and Homeless Management Information System (HMIS) documentation that clearly identifies client goals, action steps, services provided, outcomes, and required service transactions. Function as a contributing member of a multidisciplinary team with the organization’s employees, volunteers, and community agencies
    • Participate in staff meetings and trainings as directed by supervisor, including but not limited to Housing-Focused Case Management, Motivational Interviewing, Trauma-Informed Care, and Harm Reduction
    • Be on-call status as directed by supervisor
    • Adhere to the Carpenter’s Shelter Code of Ethics and SEED Values
    • Perform other duties as assigned

    Telework is eligible up to one day per week.

    REQUIRED:

    • Bachelor’s degree or equivalent experience in social work, human services or a related field
    • Ability to develop strong working relationships with households receiving services
    • Ability to maintain documentation
    • Ability to manage an independent caseload while balancing multiple priorities and deadlines.
    • Strong time management and organizational skills
    • Proficient skill level in Microsoft Office 365 (Word, Excel, Power Point, Teams, SharePoint)
    • Valid driver’s license and ability to pass a background check
    • Flexibility to work outside of normal business hours, if needed, to accommodate client and community needs

    PREFERRED:

    • Two to three years of experience in case management or supportive housing, preferably with chronically homeless or highly vulnerable populations.
    • Knowledge and experience in Northern Virginia homeless and housing resources.
    • Direct personal connection to homelessness

    SALARY: $58,000-$60,000 plus benefits (medical, dental, vision, life insurance, 403(b), PTO, sick leave, holidays)

    OPPORTUNITY to receive clinical supervision toward LCSW licensure through a qualified LCSW supervisor.
