    Description

    POSITION SUMMARY:

    The Health Coordinator supports the delivery of integrated medical and behavioral health

    services within the OASAS Part 822 Outpatient Program in accordance with 14 NYCRR 822.7

    and related clinical standards.

    The Health Coordinator assists in ensuring that required medical screening, referral coordination,

    communicable disease services, overdose prevention education, and Medication for Addiction

    Treatment (MAT) access are properly documented and integrated into the client’s treatment and

    recovery plan.

    The Health Coordinator functions under the clinical supervision of a Qualified Health

    Professional (QHP) and collaborates with the Medical Director to support compliance with 822.7 (i) medical service requirements.

    POSITION DESCRIPTION:

    In alignment with 14 NYCRR 822.7, the Health Coordinator will:

    Medical Screening & Referral Support (822.7(i))

    • Ensure documentation that each client has had a physical examination within the prior twelve(12) months, or coordinate referral for medical evaluation when required.
    • Track and document referrals for medical, psychiatric, and specialty care services.
    • Coordinate follow-up communication between counseling staff and medical providers when clinically indicated.
    • Assist the Medical Director in monitoring compliance with medical service requirements.

    Medication for Addiction Treatment (MAT) Support (822.7(i)(4); MAT Standards)

    • Ensure that all clients receive education regarding approved medications for substance use disorder treatment.
    • Assist in documenting client preference, acceptance, or refusal of MAT services.
    • Coordinate communication with Opioid Treatment Programs (OTP) pursuant to executed Memoranda of Understanding (MOU).
    • Track external MAT referrals and ensure documentation is maintained in the clinical record.

    Communicable Disease & Health Education (822.7(f))

    • Ensure completion and documentation of communicable disease risk assessments.
    • Facilitate referrals for HIV, Hepatitis C, tuberculosis, and other indicated testing.
    • Provide or coordinate health education services including overdose prevention, naloxone education, tobacco cessation, and chronic disease management.
    • Ensure naloxone education and distribution is documented in compliance with regulatory expectations.

    Treatment Planning Integration (822.7; 822.8(h))

    • Participate in multidisciplinary case conferences and treatment planning meetings.
    • Provide input regarding medical and health-related needs during treatment planning and utilization review.
    • Ensure medical and health service coordination is reflected in the treatment plan and progress notes when applicable.

    Documentation & Compliance

    • Document all services in the Electronic Health Record (EHR) in compliance with OASAS Part 822, 42 CFR Part 2, and HIPAA.
    • Participate in internal chart audits and quality assurance reviews.
    • Maintain documentation necessary to demonstrate compliance with staffing and service requirements under 822.7.

    Program Collaboration

    • Attend weekly clinical staff meetings and multidisciplinary team meetings.
    • Collaborate with the Director of Clinical Services and Medical Director regarding service delivery improvements.
    • Participate in required training and quality improvement initiatives.

    The Health Coordinator operates under:

    • Administrative supervision of the Program Director
    • Clinical oversight by the Medical Director when applicable
    • Multidisciplinary case conference review

    JOB COMPETENCY:

    • Bachelor’s degree in nursing, public health, health services administration, social work, or related field; OR licensed healthcare professional (RN, LPN, NP); OR documented experience in medical care coordination within behavioral health or substance use treatment.
    • Knowledge of OASAS Part 822 regulations preferred.
    • Understanding of Medication for Addiction Treatment (MAT) and integrated care models.
    • Ability to document services consistent with 822.7 and 822.8 standards.
    • Strong organizational and communication skills.
    • Bilingual English/Spanish preferred.
    • Must possess sight, hearing and digital senses or use prosthesis that will enable the essential functions of the job to be completed.

    This position does not independently provide services outside scope of training or licensure and

    does not perform functions requiring a Qualified Health Professional unless appropriately

    credentialed.
