    Description

    Our growing team seeks a Case Manager/Community Engagement Specialist (CM/CES) to provide services for Connectedly’s GrandFamily Resource Center (GFRC). The GFRC provides a wide range of services for grandparents raising grandchildren who are in need of support and resources. GFRC programs take a holistic, trauma-informed approach to offer grandparent-headed families mental health support, educational tools, and community resources. The CM/CES will work closely with the GFRC Coordinator to support clients with referrals to community services and resources and provide other relevant information to support the families’ needs. They will participate in community outreach and recruitment and serve as a point of contact to connect grandparents with other programs. They will have demonstrated experience working with parenting adults and/or older adults in an urban setting using a trauma-informed, culturally and linguistically sensitive approach. They will hold at minimum a bachelor’s degree, preferably in social work or a related field, and have a minimum of two years’ experience in social services. A licensed driver with their own vehicle is strongly preferred as this position requires regular travel throughout Philadelphia. This is a full-time position at 40 hours a week and is scheduled during Connectedly’s standard operating hours, Monday – Friday, 9 AM – 5 PM. Occasional evenings and weekends are required for special events. Connectedly provides a hybrid work model and an employee benefits package.

    The CM/CES will:

    • Work closely with the GFRC Coordinator to assess clients' physical and mental wellness, family needs, preferences, and abilities
    • Listen to clients’ concerns and coordinate referrals as needed to mental health, physical health, housing, financial, legal, and educational services, and other community services as appropriate
    • Stays up to date on resources and services that can support grandfamilies in GFRC programming
    • Follow up with clients to ensure services and supports have been accessed; problem-solve any concerns/issues
    • Provide occasional facilitator coverage for Connectedly groups.
    • Develop and maintain relationships within targeted Philadelphia neighborhoods to build interest about Connectedly’s GFRC programs
    • Engage in outreach to community organizations, schools, family service programs, and other educational/human service programs to enroll families in Connectedly's GFRC program
    • Attend local community events to educate community on Connectedly's programming and obtain program referrals.
    • Oversee Connectedly’s Grandparent Ambassadors team of outreach volunteers
    • Develop marketing materials, such as flyers and brochures
    • Collaborate with GFRC Coordinator to:
    • Implement community-based workshop sessions, as needed, that help grandparents build caregiving skills to support their grandchildren
    • Develop, coordinate, and facilitate interactive special family events that strengthen grandfamilies’ development and growth
    • Recruit volunteer professionals to coach grandparents in the program’s special topic workshops
