    Residence Counselor - Hope House on Crotona Park

    Job Summary:

    The Residence Counselor for Hope House on Crotona Park (HHCP), a residential alternative-to-incarceration, is responsible for providing direct clinical services, complex care management, and discharge planning to residents of the Hope House program — a justice-involved, high-order felony offending population living with Serious Mental Illness (SMI) and, in many cases, co-occurring substance use disorders.

    Working within a Work-Ordered Day model, the Residence Counselor delivers individualized, trauma-informed, and recovery-oriented services designed to promote healing. stability, personal growth, and successful reintegration into the community. The Residence Counselor provides ongoing therapeutic support and ensures that all care aligns with OMH regulations, HHCP policies and procedures, and each client’s court-approved treatment plan.

    The Residence Counselor reports directly to the Residence Supervisor and participates in weekly clinical supervision and interdisciplinary team meetings to ensure comprehensive, integrated, and coordinated care.

    Duties include, but are not limited to:

    1. Provide individual counseling and complex care management services to an assigned cohort of up to eight (8) residents.
    2. Deliver services, supports, and interventions that are consistent with the principles of Trauma-Informed Care to prevent re-traumatization and promote healing.
    3. Develop and maintain individualized service plans collaboratively with residents that reflect personal treatment goals, stages of change, and engagement readiness.
    4. Conduct comprehensive assessments, including psychosocial, mental health, and risk assessments, in accordance with OMH and agency guidelines.
    5. Maintain accurate and timely clinical documentation, including progress notes, service plans, and discharge summaries, consistent with OMH Part 595 regulations and program standards.
    6. Coordinate services with the interdisciplinary team, including clinical, residential, medical, and peer staff, to ensure integrated and effective care.
    7. Support residents in identifying barriers to engagement and assist in developing coping and problem-solving strategies.
    8. Facilitate group counseling sessions, psychoeducational activities, and skill-building workshops as assigned.
    9. Participate actively in weekly clinical supervision and interdisciplinary case conferences with the Program Director and staff from clinical and medical provider partners.
    10. Develop and implement discharge and aftercare plans that promote continuity of care and successful community reintegration.
    11. Respond to crises in collaboration with the Residence Supervisor and Program Director, employing de-escalation and risk assessment techniques as needed.
    12. Ensure that residents’ rights are upheld and that services are delivered in a manner that promotes dignity, empowerment, and self-determination.
    13. Participate in ongoing staff training, supervision, and professional development to maintain compliance with agency standards and best practices.

    Minimum qualifications:

    1. Must be a Licensed Mental Health Counselor (LMHC) in the State of New York or an equivalent qualified mental health professional as determined by the Greenburger Center and Executive Staff.
    2. Must have a minimum of two (2) years’ experience working with individuals diagnosed with Serious Mental Illness (SMI) and/or co-occurring substance use disorders, preferably in a residential or therapeutic community setting.
    3. Demonstrated ability to perform comprehensive assessments, develop service plans, and provide individual counseling and case management.
    4. Knowledge of OMH Part 595 regulations and evidence-based practices in recovery-oriented, trauma-informed care.
    5. Strong written and verbal communication skills, with proficiency in documentation and record-keeping systems.
    6. Must be self-aware and possess effective interpersonal and problem-solving skills, with the ability to engage clients respectfully and therapeutically.
    7. Must be reliable, self-motivated, and capable of working both independently and collaboratively within a multidisciplinary team.
    8. Must be computer literate and comfortable using electronic medical record (EMR) systems.
    9. Must be available to work flexible hours, including evenings, weekends, and holidays, as the program requires.

    The essential functions of the job include:

    1. Providing direct trauma-informed therapeutic and case management services to assigned residents.
    2. Developing, documenting, and monitoring individualized treatment and discharge plans.
    3. Participating in supervision and interdisciplinary collaboration to ensure integrated and coordinated, person-centered care.
    4. Maintaining consistent attendance, reliability, and professionalism.
